【摘要】：目的探討右美托咪定與咪達(dá)唑侖(力月西)在胸科老年人麻醉術(shù)后譫妄的臨床效果。方法將76例胸外科老年手術(shù)患者隨機(jī)分為觀察組和對(duì)照組各38例,兩組均給予芬太尼持續(xù)靜脈泵入鎮(zhèn)痛,對(duì)照組首次靜脈推注力月西0.05mg/kg,隨后以0.02~0.08 mg·kg~(-1)·h~(-1)維持麻醉,觀察組首次靜脈推注右美托咪定1.0μg/kg,然后以0.2~0.7μg·kg~(-1)·h~(-1)維持麻醉,比較兩組鎮(zhèn)靜效果和不良反應(yīng)發(fā)生。結(jié)果兩組鎮(zhèn)靜效果時(shí)間均無統(tǒng)計(jì)學(xué)差異(P0.05);但觀察組停藥后蘇醒時(shí)間短于對(duì)照組,不良反應(yīng)發(fā)生率低于對(duì)照組(P0.05)。兩組手術(shù)后SDS及SAS評(píng)分明顯低于干預(yù)前,且觀察組明顯低于對(duì)照組(P0.05)。結(jié)論右美托咪定用于胸外科老年手術(shù)中的鎮(zhèn)靜效果好,且降低不良反應(yīng)發(fā)生率。
[Abstract]:Objective to investigate the clinical effect of dexmetomidine and midazolam on delirium after thoracic anesthesia in the elderly. Methods Seventy-six elderly patients undergoing thoracic surgery were randomly divided into observation group (n = 38) and control group (n = 38). Fentanyl was administered with continuous intravenous infusion of fentanyl in both groups. The control group received intravenous injection of 0.05 mg / kg for the first time, and then maintained anesthesia with 0.02g / kg 0.08 mg kg~ (-1) h ~ (-1). In the observation group, dexmetomidine was given intravenously for the first time, and then maintained with 0.2 渭 g kg~ (-1) h-1. The sedation effect and adverse reactions were compared between the two groups. Results there was no significant difference in sedation time between the two groups (P0.05), but the recovery time of the observation group was shorter than that of the control group, and the incidence of adverse reactions was lower than that of the control group (P0.05). The scores of SDS and SAS in the two groups after operation were significantly lower than those before intervention, and those in the observation group were significantly lower than those in the control group (P0.05). Conclusion the sedative effect of dexmetomidine in elderly thoracic surgery is good and the incidence of adverse reactions is decreased.
