【摘要】：目的比較局部麻醉和全身麻醉對肺癌患者血清IL-4、IFN-γ和IL-18水平的影響。方法選取筆者所在醫(yī)院收治的肺癌患者80例,根據(jù)不同的麻醉方法隨機分為局部麻醉組和全身麻醉組,每組40例。局部麻醉組行硬膜外麻醉,全身麻醉組行靜吸復(fù)合全麻,分別使用酶聯(lián)免疫吸附試驗(ELISA)測定兩組患者在麻醉誘導(dǎo)前(T0)、手術(shù)切皮(T1)和手術(shù)開始后1 h(T2)的血清細胞因子白細胞介素-4(IL-4)、干擾素-γ(IFN-γ)和白細胞介素-18(IL-18)的水平。結(jié)果局部麻醉組患者在T1和T2時血清中IL-4、IFN-γ和IL-18水平明顯高于T0時,差異具有統(tǒng)計學(xué)意義(P0.05);全身麻醉組患者在T2時血清中IL-4、IFN-γ和IL-18水平與T0時比較,差異沒有統(tǒng)計學(xué)意義(P0.05)。T1、T2時,局部麻醉組患者血清中IL-4、IFN-γ和IL-18水平均明顯高于全身麻醉組,差異具有統(tǒng)計學(xué)意義(P0.05)。結(jié)論硬膜外麻醉能夠明顯提高肺癌患者血清中IL-4、IFN-γ和IL-18水平,有利于保護患者的免疫系統(tǒng),提高腫瘤患者的抗炎能力。
[Abstract]:Objective to compare the effects of local anesthesia and general anesthesia on serum IL-4,IFN- 緯 and IL-18 levels in patients with lung cancer. Methods 80 patients with lung cancer were randomly divided into local anesthesia group and general anesthesia group according to different anesthetic methods. The local anesthesia group received epidural anesthesia, and the general anesthesia group received general anesthesia combined with intravenous inhalation. The levels of serum cytokines interleukin-4 (IL-4), interferon- 緯 (IFN- 緯) and interleukin-18 (IL-18) before anesthesia induction (T0), skin incision (T1) and 1 hour (T2) after operation were measured by Elisa (ELISA). Results the serum levels of IL-4,IFN- 緯 and IL-18 in local anesthesia group were significantly higher than those in T 0 group (P0.05), but there was no significant difference in IL-4,IFN- 緯 and IL-18 levels in T 2 group and T 0 group (P0.05). The serum levels of IL-4,IFN- 緯 and IL-18 in local anesthesia group were significantly higher than those in general anesthesia group (P0.05). Conclusion epidural anesthesia can significantly increase the levels of IL-4,IFN- 緯 and IL-18 in the serum of patients with lung cancer, protect the immune system and improve the anti-inflammatory ability of patients with lung cancer.
