【摘要】：目的:研究全髖關(guān)節(jié)置換術(shù)后假體脫位的原因分析及處理對策。方法:本次研究對象2015年1月-2015年12月間在我院接受治療的全髖關(guān)節(jié)置換患者80例,對術(shù)后脫位的4例患者的臨床資料進(jìn)行系統(tǒng)的分析,分析脫位的原因和處理措施。結(jié)果:4例脫位患者中,初次全髖關(guān)節(jié)置換術(shù)后脫位2例、翻修手術(shù)后脫位2例;既往髖部手術(shù)史患者3例;患者中腦部手術(shù)后出現(xiàn)精神異常1例,同時(shí)不配合本次治療;所有患者的脫位方向均為后脫位;4例脫位患者均首先在麻醉下進(jìn)行閉合復(fù)位,下肢采用牽引治療,時(shí)間為21天左右,3例患者牽引治療后獲得穩(wěn)定,并且未再出現(xiàn)復(fù)發(fā)性脫位,1例患者在牽引治療后的90天內(nèi)繼發(fā)2次脫位,同時(shí)在復(fù)位中出現(xiàn)髖臼松動采用手術(shù)翻修髖臼調(diào)整外展角后獲得穩(wěn)定。結(jié)論:由于多種原因所引發(fā)的全髖關(guān)節(jié)置換術(shù)后假體脫位,護(hù)理人員須認(rèn)真分析脫位原因,對患者加強(qiáng)護(hù)理,患者需臥床制動。
[Abstract]:Objective: to study the causes of dislocation of prosthesis after total hip arthroplasty (THR). Methods: from January 2015 to December 2015, 80 patients with total hip replacement received treatment in our hospital. The clinical data of 4 patients with postoperative dislocation were analyzed systematically, the causes of dislocation and the treatment measures were analyzed. Results among 4 cases of dislocation, 2 cases were dislocation after primary total hip replacement, 2 cases were dislocation after revision operation, 3 cases had previous history of hip surgery, 1 case had mental abnormality after brain operation, and 1 case did not cooperate with this treatment. The dislocation direction of all the patients was that 4 patients with posterior dislocation were treated with closed reduction under anesthesia, and the lower extremities were treated with traction. The time of traction treatment was 21 days or so in 3 patients, and the stability was obtained after traction treatment. There was no recurrence of recurrent dislocation in one case, and the dislocation occurred twice within 90 days after traction treatment, and the acetabular loosening occurred during the reduction, and the acetabular angle was adjusted by surgical revision. Conclusion: because of the dislocation of prosthesis after total hip replacement caused by various reasons, the nursing staff should carefully analyze the reasons of dislocation, strengthen the nursing care of the patients, and the patients should stay in bed and brake.
