【摘要】：目的研究小兒眼科手術(shù)麻醉誘導(dǎo)時(shí)應(yīng)用肌松劑對(duì)喉罩置入的影響。方法選擇眼科患兒60例(先天性斜視40例,先天性白內(nèi)障10例,先天性青光眼10例),年齡2~8歲,隨機(jī)分為兩組,每組30例。實(shí)驗(yàn)組誘導(dǎo)時(shí)給予適量肌松劑(麻醉中不再追加)后置入喉罩,對(duì)照組不給予肌松劑。記錄兩組患兒誘導(dǎo)前(T_0)、誘導(dǎo)后(T_1)的心率及置入喉罩時(shí)(T_2)心率變化,嗆咳、體動(dòng)發(fā)生例數(shù)。分別記錄兩組術(shù)后呼吸恢復(fù)、拔除喉罩時(shí)間及拔除后(T_3)的心率。結(jié)果兩組患兒誘導(dǎo)后心率較誘導(dǎo)前均有明顯下降(P0.05)。置入喉罩時(shí)實(shí)驗(yàn)組患兒生命體征平穩(wěn),無嗆咳、體動(dòng)。對(duì)照組有6例出現(xiàn)體動(dòng)、嗆咳,1例出現(xiàn)了氣道痙攣,需要停止操作追加誘導(dǎo)用藥后完成。術(shù)畢在呼吸恢復(fù)拔出喉罩時(shí)間上兩組無明顯差異(P0.05)。結(jié)論對(duì)于小兒眼科手術(shù),全麻誘導(dǎo)時(shí)合用肌松劑可使喉罩置入更順暢,且不延長術(shù)后蘇醒及拔除喉罩時(shí)間,應(yīng)用更加安全。
[Abstract]:Objective to study the effect of muscle relaxant on laryngeal mask implantation during anesthesia induction in pediatric ophthalmic surgery. Methods 60 cases of ophthalmic children (40 cases of congenital strabismus, 10 cases of congenital cataract and 10 cases of congenital glaucoma) were randomly divided into two groups, 30 cases in each group. The experimental group was given a proper amount of muscle relaxant (no longer added during anesthesia) and the control group was not given a muscle relaxant. The changes of heart rate, cough and body movement were recorded in both groups before and after induction (T _ s _ 1) and after placement of larynx mask (T _ 2). Respiratory recovery, laryngeal mask extraction time and heart rate (T _ 3) after extraction were recorded in both groups. Results after induction, the heart rate in both groups was significantly lower than that before induction (P0.05). When the laryngeal mask was placed, the vital signs of the children in the experimental group were stable, no cough and body movement. In the control group, 6 cases developed body movement and 1 case developed airway spasm. There was no significant difference between the two groups in the time of respiratory recovery and extraction of larynx mask after operation (P0.05). Conclusion for pediatric ophthalmic surgery, combined with muscle relaxant during general anesthesia induction can make laryngeal mask placement more smooth, without prolonging postoperative recovery and removing larynx mask time, and it is more safe to use.
