【摘要】：目的觀察側(cè)隱窩、椎間孔注藥聯(lián)合推拿療法治療腰椎間盤突出癥的效果。方法腰椎間盤突出癥患者300例,隨機(jī)分為三組,各100例。A組采用側(cè)隱窩和椎間孔注藥(配方:康寧克通A 10 mg,利多卡因60 mg,維生素B121 000μg,加生理鹽水至8 m L)聯(lián)合推拿療法(腰部按壓、斜扳、直腿抬高),B組僅予側(cè)隱窩和椎間孔注藥治療,C組單用推拿療法。治療后3~4周計(jì)算優(yōu)良率及治愈率,記錄A、B組局部注藥次數(shù)及藥物用量。分別于治療后3、6、12個(gè)月觀察復(fù)發(fā)情況。記錄治療期間的并發(fā)癥及不良反應(yīng)發(fā)生情況。結(jié)果治療后3~4周,A組治愈率、優(yōu)良率高于B、C組(P均0.05)。A組注藥次數(shù)及康寧克通A用量低于B組(P均0.05)。治療后6、12個(gè)月,A、C組復(fù)發(fā)率低于B組(P均0.05)。A、B組注藥時(shí)均未出現(xiàn)全脊髓麻醉、馬尾綜合征及肌無(wú)力等神經(jīng)損傷現(xiàn)象。C組治療后3例出現(xiàn)腰痛加重,經(jīng)臥床休息2~3 h恢復(fù)到治療前。結(jié)論側(cè)隱窩、椎間孔注藥聯(lián)合推拿治療腰椎間盤突出癥療效較好,復(fù)發(fā)率低。
[Abstract]:Objective to observe the effect of lateral recess, intervertebral foramen injection and massage therapy on lumbar disc herniation. Methods 300 patients with lumbar disc herniation were randomly divided into three groups. Group A (n = 100) was treated with lateral recess and intervertebral foramen injection (formulation: Kangninkitong A 10 mg, lidocaine 60 mg, vitamin B12 21 000 渭 g, normal saline to 8 mL). Group B was treated with only massage therapy for lateral recess and intervertebral foramen. The excellent and good rate and cure rate were calculated 3 ~ 4 weeks after treatment, and the times of local injection and the dosage of drugs were recorded. Recurrence was observed at 6 and 12 months after treatment. The complications and adverse reactions during treatment were recorded. Results 3 weeks after treatment, the cure rate of group A was higher than that of group B (P 0.05). The times of injection and the dosage of Kangning Ketong A in group A were lower than those in group B (P 0.05). The recurrence rate in group C was lower than that in group B (P 0.05). There was no total spinal cord anesthesia in group A (P 0.05). The symptoms of nerve injury such as cauda equina syndrome and myasthenia were aggravated in group C. after treatment, the recurrence rate in group C was significantly lower than that in group B (P 0.05). Conclusion the treatment of lumbar disc herniation with intervertebral foramen injection combined with massage is effective and the recurrence rate is low.
