【摘要】：目的探討持續(xù)泵注舒芬太尼在老年婦科患者開腹手術中的安全性和有效性。方法 80例ASAⅡ-Ⅲ級擇期行婦科開腹手術患者,年齡60~85歲,隨機分為兩組:R組40例采用瑞芬太尼6μg/kg·h;S組40例采用舒芬太尼0.1μg/kg·h。于麻醉誘導氣管插管后吸入1~3%七氟烷維持麻醉,根據患者入組情況分別持續(xù)泵注瑞芬太尼和舒芬太尼,于手術結束前10分鐘停止泵注鎮(zhèn)痛藥。記錄患者的一般情況、生命體征變化情況,麻黃堿、阿托品的用量,記錄患者術后呼吸和意識恢復情況、蘇醒后的疼痛評分及患者蘇醒即刻躁動的發(fā)生率。結果低血壓和心率減慢的發(fā)生率在S組為37.5%和20%均低于R組的60.5%和62.5%,差異有統(tǒng)計學意義(P0.05);相應麻黃堿和阿托品的用量S組為(5.48±2.27)和(0.33±0.12)也明顯低于R組的(12.4±3.23)和(0.81±0.13),差異有統(tǒng)計學意義(P0.05);呼吸恢復時間和潮氣量恢復時間R組為(4.3±2.2)min和(5.6±1.5)min均短于S組的(6.3±3.4)min和(8.1±2.5)min,差異有統(tǒng)計學意義(P0.05);但兩組意識恢復時間差異無統(tǒng)計學意義(P0.05);疼痛評分R組明顯高于S組,分別為(6.5±2.6)分和(4.1±1.3)分(P0.05);蘇醒即刻躁動發(fā)生率S組明顯低于R組,分別為12.5%和55%,差異有統(tǒng)計學意義(P0.05)。結論舒芬太尼持續(xù)泵注具有血流動力學穩(wěn)定、術后恢復快、不良反應少、鎮(zhèn)痛作用較強等優(yōu)點,可安全用于老年患者婦科開腹手術的麻醉維持。
[Abstract]:Objective to explore the safety and effectiveness of sufentanil in open surgery for elderly gynecologic patients. Methods 80 ASA II - III gynecologic open surgery patients, aged 60~85 years, were randomly divided into two groups: 40 cases in group R using remifentanil 6 g/kg h; 40 cases in group S after sufentanil 0.1 g/kg. H. after anesthesia induction tracheal intubation 1~3% seven halothane was inhaled to maintain anesthesia. The patients were pumped by sufentanil and sufentanil, respectively, and 10 minutes before the operation. The general condition of the patients, the changes of vital signs, the dosage of ephedrine, atropine, the recovery of postoperative respiration and consciousness, and the pain evaluation after awakening were recorded. The difference was statistically significant (P0.05). Conclusion sufentanil continuous pump has a stable hemodynamics, quick recovery, less adverse reaction and stronger analgesic effect. It can be used safely for the maintenance of anesthesia in elderly patients with gynecologic open surgery.
