【摘要】：目的:探討右美托咪定對七氟烷麻醉下不停跳冠狀動脈旁路移植術(shù)患者血流動力學(xué)的影響。方法:選取2013年10月—2014年11月鄭州市中醫(yī)院收治的行不停跳冠狀動脈旁路移植術(shù)的患者112例,以隨機(jī)數(shù)字表法分為觀察組和對照組各56例。對照組給予0.9%氯化鈉注射液,觀察組給予右美托咪定。觀察2組患者橈動脈穿刺完畢(T_0)、氣管插管完畢(T_1)、劈開胸骨(T_2)、搭前降支(T_3)、頭低位搭后降支(T_4)、術(shù)畢(T_5)時各項指標(biāo)情況。結(jié)果:觀察組患者在T_1、T_2、T_4、T_5時的平均動脈壓均明顯低于對照組,差異均有統(tǒng)計學(xué)意義(P0.05);觀察組患者在T_1、T_2、T_4、T_5時的心率均明顯低于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:在七氟烷麻醉下不停跳冠狀動脈旁路移植術(shù)中持續(xù)泵注右美托咪定,療效較好,應(yīng)激反應(yīng)減輕,平均動脈壓降低,改善心率,值得推廣。
[Abstract]:Aim: to investigate the effect of dexmetomidine on hemodynamics in patients undergoing coronary artery bypass grafting (CABG) under sevoflurane anesthesia. Methods: 112 patients treated with coronary artery bypass grafting in Zhengzhou traditional Chinese Medicine Hospital from October 2013 to November 2014 were randomly divided into observation group (n = 56) and control group (n = 56). The control group was given 0.9% sodium chloride injection, and the observation group was given dexmetomidine. The indexes of the two groups were observed at the end of the puncture of radial artery, the tipping of T _ (1), the splitting of sternum T _ (2), the establishment of anterior descending branch (T _ (3), the first and posterior branch of T _ (4), and the end of operation (T _ (5). Results: the mean arterial pressure of the patients in the observation group was significantly lower than that in the control group at T _ 1 / T _ 2 / T _ 4 / T _ 5, and the heart rate in the observation group was significantly lower than that in the control group at the time of T _ 1 / T _ 2 / T _ 4 / T _ 5, and the difference was statistically significant (P 0.05). Conclusion: continuous pump of dexmetomidine in coronary artery bypass grafting under sevoflurane anesthesia has better effect, less stress response, lower mean arterial pressure, improved heart rate and is worth popularizing.
