【摘要】：正患者,男,22歲,因右腕背刀砍傷2h于2015年1月17日入院。體格檢查:右腕背橈側(cè)斜行約3.5 cm左右傷口,邊緣整齊,輕度污染,肌腱斷端外露。拇指掌指關(guān)節(jié)活動(dòng)稍受限,指間關(guān)節(jié)伸屈功能無(wú)異常,外展功能稍限。手背橈側(cè)和橈側(cè)2個(gè)半手指背側(cè)指間關(guān)節(jié)近端的皮膚感覺(jué)麻木。橈動(dòng)脈搏動(dòng)良好,其余各指運(yùn)動(dòng)及感覺(jué)功能無(wú)異常。各指末梢血運(yùn)充盈良好。送手術(shù)室在臂叢麻醉下,手術(shù)探查發(fā)現(xiàn):伸肌支持帶斜行斷裂,拇長(zhǎng)展肌腱、拇短伸肌腱及橈神經(jīng)淺支斷裂,橈側(cè)腕長(zhǎng)、腕短伸肌腱及正常拇長(zhǎng)伸肌腱未見(jiàn)斷裂,另發(fā)現(xiàn)傷口遠(yuǎn)側(cè)端有一肌腱與拇短伸肌腱伴行,肌鍵寬度、厚度與正常拇長(zhǎng)伸肌腱相當(dāng)(見(jiàn)圖1)。牽拉
[Abstract]:The patient, 22 years old, was admitted to hospital on January 17, 2015 for 2 hours with a right wrist knife. Physical examination: the right dorsal radial side of the right wrist oblique about 3.5 cm about the wound, the edge is neat, slightly contaminated, the tendon was exposed at the broken end. The movement of metacarpophalangeal joint of thumb was limited, the extension and flexion function of interphalangeal joint was not abnormal, and the abduction function was limited. Skin numbness at the proximal end of the dorsal interphalangeal joint of the dorsal and radial sides of the hand. The radial artery pulsated well and the other fingers had no abnormal motor and sensory function. The end of each finger blood flow is good. Under brachial plexus anaesthesia, the surgical exploration found that the extensor spurs had oblique rupture, the extensor pollicis longus tendon, the extensor pollicis brevis tendon and the superficial branch of the radial nerve, the length of the radial wrist, the extensor carpi brevis tendon and the normal extensor pollicis longus tendon were not broken. It was also found that there was a tendon at the distal end of the wound accompanied by the extensor pollicis brevis tendon, and the width and thickness of the muscle bond were similar to those of the normal extensor pollicis longus tendon (see figure 1).
