【摘要】：目的探討小兒小腸疝氣伴上呼吸道感染行腹腔鏡手術的麻醉選擇。方法將100例擬接受腹腔鏡手術治療的小腸疝氣伴上呼吸道感染患兒按照隨機數(shù)字表法分成對照組與觀察組,每組50例。對照組給予以氯胺酮為主的基礎麻醉,觀察組給予全憑七氟烷吸入麻醉。比較兩組睫毛反射消失時間、完全清醒時間、手術時間、呼吸道分泌物增加等情況。結果觀察組完全清醒時間顯著低于對照組,觀察組呼吸道分泌物增加不良反應發(fā)生率為6.0%(3/50),明顯低于對照組的24.0%(12/50)(P0.05)。兩組睫毛反射消失時間、手術時間比較,差異無統(tǒng)計學意義(P0.05)。結論在小腸疝氣行腹腔鏡手術治療的伴上呼吸道感染患兒中選擇全憑七氟烷吸入麻醉,能有效減少呼吸道的刺激,有助于手術的順利進行,術后意識恢復快、安全性高,值得臨床廣泛應用。
[Abstract]:Objective to investigate the anesthetic choice of laparoscopic surgery for small intestinal hernia with upper respiratory tract infection in children. Methods 100 children with intestinal hernia associated with upper respiratory tract infection were divided into control group (n = 50) and observation group (n = 50). The control group was given ketamine-based anesthesia and the observation group was given total sevoflurane inhalation anesthesia. The time of disappearance of eyelash reflex, the time of complete wakefulness, the time of operation and the increase of respiratory tract secretion were compared between the two groups. Results the total waking time in the observation group was significantly lower than that in the control group. The incidence of adverse reactions in the observation group was 6. 0 / 50, which was significantly lower than that in the control group (24. 0 / 12 / 50) and the incidence of adverse reactions was 0. 05% (P < 0. 05). There was no significant difference in the time of disappearance of eyelash reflex and operative time between the two groups (P 0.05). Conclusion the total sevoflurane inhalation anesthesia in children with upper respiratory tract infection after laparoscopic operation for small intestinal hernia can effectively reduce respiratory irritation, facilitate the smooth progress of operation, recover consciousness quickly and have high safety after operation. It is worthy of extensive clinical application.
