【摘要】：目的肺隔離征是一種先天性肺發(fā)育畸形,該病發(fā)病率低、誤診率較高。文中結(jié)合肺隔離征患者臨床資料,對(duì)該病的診斷與治療進(jìn)行總結(jié)分析。方法回顧性分析南京軍區(qū)南京總醫(yī)院心胸外科2009年9月至2014年9月收治的18例肺隔離征患者臨床資料,其中男8例、女10例,年齡20~68歲,平均(42.37±14.63)歲。所有患者均行影像學(xué)檢查,主要表現(xiàn)為腫塊影(n=11)及囊性改變(n=7);均采取雙腔氣管內(nèi)插管靜脈復(fù)合麻醉,并行手術(shù)切除。結(jié)果 17例葉內(nèi)型行病變肺葉切除術(shù),另1例葉外型行肺局部隔離病變切除術(shù)。明確診斷14例、誤診3例、漏診1例。18例患者經(jīng)手術(shù)治療全部康復(fù)出院,隨訪8~18個(gè)月,無失訪。患者生存良好,未出現(xiàn)與手術(shù)相關(guān)的并發(fā)癥。結(jié)論肺隔離征臨床表現(xiàn)缺乏特異性,分類診斷相對(duì)困難,手術(shù)治療為主,術(shù)后預(yù)后良好。
[Abstract]:Objective Pulmonary sequestration is a congenital deformity of lung, with low incidence and high misdiagnosis rate. The diagnosis and treatment of pulmonary sequestration syndrome were summarized and analyzed. Methods the clinical data of 18 patients with pulmonary sequestration from September 2009 to September 2014 in the Department of Cardiothoracic surgery, Nanjing General Hospital of Nanjing military region were retrospectively analyzed, including 8 males and 10 females, aged 20 to 68 years, with an average of 42.37 鹵14.63 years old. All the patients underwent imaging examination, the main manifestations of which were mass shadow 11) and cystic change. All patients were treated with double-lumen endotracheal intubation combined with intravenous anesthesia and surgical resection. Results 17 cases of intra-lobar type underwent pulmonary lobectomy and 1 case of extralobar type underwent partial pulmonary sequestration. 14 cases were definitely diagnosed, 3 cases were misdiagnosed, and 1 case was missed. 18 cases were all recovered and discharged after operation. The follow-up was 8 ~ 18 months. The patient survived well and had no surgical complications. Conclusion the clinical manifestations of pulmonary sequestration are lack of specificity, classification diagnosis is relatively difficult, surgical treatment is the main, and the postoperative prognosis is good.
