【摘要】：目的研究不同全麻方式對(duì)腹腔鏡結(jié)腸癌切除術(shù)患者圍麻醉期T淋巴細(xì)胞亞群的影響。方法選取2008年3月-2012年6月于該院接受腹腔鏡結(jié)腸癌切除術(shù)的患者80例,按照全麻方式的不同分為3組,接受靜脈全麻的患者共28例(A組);接受吸入全麻的患者共25例(B組);接受靜吸復(fù)合全麻的患者共27例(C組)。檢測(cè)3組患者圍麻醉期的T淋巴細(xì)胞亞群水平并進(jìn)行對(duì)照研究。結(jié)果 3組患者全麻后均出現(xiàn)CD3~+、CD4~+和CD8~+水平降低,B組的CD4~+/CD8~+和NK細(xì)胞水平均明顯降低,C組的CD4~+/CD8~+明顯降低。B組患者T1、T2的各指標(biāo)均明顯低于A組,C組患者T1的CD3~+和CD8~+水平明顯低于A組,T2除NK細(xì)胞外其他各指標(biāo)均明顯低于A組,另外C組患者T1的CD8~+明顯低于B組,NK細(xì)胞明顯高于B組,T2的CD4~+明顯高于B組。結(jié)論靜脈全麻對(duì)于腹腔鏡結(jié)腸癌切除術(shù)患者圍麻醉期T淋巴細(xì)胞亞群影響最小,對(duì)免疫功能的影響最為輕微。
[Abstract]:Objective to study the effect of different general anesthesia methods on T lymphocyte subsets in patients undergoing laparoscopic colon cancer resection. Methods from March 2008 to June 2012, 80 patients undergoing laparoscopic colon cancer resection in our hospital were divided into 3 groups according to different general anesthesia methods. A total of 28 patients received intravenous general anesthesia, 25 patients received general anesthesia, 25 patients received general anesthesia, and 27 patients received intravenous anesthesia combined with general anesthesia. The T lymphocyte subsets were measured in 3 groups of patients during the perianaesthesia period and a control study was carried out. Results after general anesthesia, the levels of CD3 ~ + CD4 ~ and CD8- ~ in group B were significantly lower than those in group A, and the levels of CD4 ~ / CD8 ~ and NK cells in group B were significantly lower than those in group A and group C. the T _ 1T _ 2 levels in group B were significantly lower than those in group A and C, respectively. The level of CD8 ~ was significantly lower than that of group A except NK cells, and the level of CD8 ~ was significantly lower than that of group A. In addition, the CD8- ~ of T _ 1 in group C was significantly lower than that in group B, which was significantly higher than that in group B and T _ 2. Conclusion intravenous general anesthesia has the least effect on T lymphocyte subsets and the least effect on immune function in patients undergoing laparoscopic colon cancer resection.
