【摘要】：目的研究硬膜外麻醉對老年糖尿病患者血液流變學、血漿胰島素水平及血糖水平的影響。方法 2014年6月至2015年6月該院收治的糖尿病患者按年齡分為老年組和非老年組,每組40例,均實施硬膜外麻醉,比較相同麻醉方式對兩組患者血液流變學指標的影響;分別對兩組患者在進行麻醉之前、手術(shù)進行30 min、手術(shù)結(jié)束時、術(shù)后4 h抽取的靜脈血進行血漿胰島素及血糖測定。結(jié)果兩組患者麻醉后全血低切和高切黏度、低切和高切還原黏度均比麻醉之前低;老年組患者麻醉后全血高切黏度比非老年組低。兩組患者麻醉前胰島素水平及血糖水平無統(tǒng)計學差異(P0.05);手術(shù)30 min、術(shù)畢、術(shù)后4 h空腹血漿胰島素血糖水平較術(shù)前升高(P0.05),與非老年組相比老年組增高較明顯。結(jié)論對糖尿病患者采用硬膜外麻醉方式可以使血液流變學相關參數(shù)降低,血漿胰島素水平及血糖水平保持相對穩(wěn)定,對臟器的血液灌注有明顯的益處,其中對老年患者效果更加顯著。
[Abstract]:Objective to study the effects of epidural anesthesia on hemorheology, plasma insulin level and blood glucose level in elderly diabetic patients. Methods from June 2014 to June 2015, diabetic patients in our hospital were divided into two groups according to their age. 40 cases in each group were treated with epidural anesthesia. The effects of the same anesthesia on hemorheology were compared between the two groups. The plasma insulin and blood glucose were measured 4 hours after the operation and 30 minutes before anesthesia. Results the whole blood low shear viscosity and high shear viscosity, low shear and high shear reduction viscosity were lower in the two groups than before, and the whole blood high shear viscosity in the elderly group was lower than that in the non-elderly group after anesthesia. There was no significant difference in insulin level and blood glucose level between the two groups before anesthesia (P 0.05), but the fasting plasma insulin and blood glucose level in 30 min, 4 h after operation was higher than that before operation (P 0.05), which was significantly higher than that in the non-elderly group. Conclusion epidural anesthesia can decrease the parameters of hemorheology and keep the level of plasma insulin and blood sugar stable, which is beneficial to the blood perfusion of viscera. Among them, the effect on the elderly patients is more remarkable.
