【摘要】：目的測(cè)定硫酸鎂聯(lián)合復(fù)方利多卡因乳膏對(duì)丙泊酚麻醉下瑞芬太尼抑制雙腔支氣管插管反應(yīng)的半數(shù)有效量(ED50)。方法選擇擇期行開(kāi)胸手術(shù)的患者,誘導(dǎo)前緩慢靜注硫酸鎂30mg/kg,其后以15mg·kg-1·h-1速率靜脈輸注直至插管完畢,雙腔支氣管導(dǎo)管囊套及前2/3均勻涂抹復(fù)方利多卡因乳膏2~3g,依次靜注丙泊酚2.0mg/kg、順苯磺酸阿曲庫(kù)銨0.3mg/kg及瑞芬太尼進(jìn)行麻醉誘導(dǎo)并插入雙腔支氣管導(dǎo)管,觀察插管反應(yīng)的發(fā)生情況。瑞芬太尼的劑量按序貫法確定,相鄰劑量的比值為1.2。結(jié)果瑞芬太尼抑制插管反應(yīng)的ED50為0.91μg/kg,95%可信區(qū)間為0.80~1.04μg/kg。結(jié)論硫酸鎂聯(lián)合復(fù)方利多卡因乳膏在丙泊酚麻醉下,瑞芬太尼抑制雙腔支氣管插管反應(yīng)的ED50為0.91μg/kg,95%可信區(qū)間為0.80~1.04μg/kg。
[Abstract]:Objective to determine the half effective dose of magnesium sulfate combined with compound lidocaine cream for remifentanil in inhibiting double lumen bronchial intubation under propofol anesthesia. Methods the patients undergoing elective thoracotomy were treated with 30 mg / kg magnesium sulfate before induction, followed by intravenous infusion of magnesium sulfate at the rate of 15mg kg-1 h-1 until intubation was completed. Double lumen bronchial duct capsule and compound lidocaine cream were evenly smeared with compound lidocaine cream for 2g / 3g before 2 / 3, followed by intravenous injection of propofol 2.0 mg / kg, 0.3mg/kg and remifentanil respectively for anesthesia induction and insertion of double-lumen bronchial catheter to observe the occurrence of intubation reaction. The remifentanil dose was determined by sequential method and the ratio of adjacent doses was 1.2. Results the ED50 of remifentanil in inhibiting intubation response was 0.91 渭 g / kg ~ 95% CI = 0.800.04 渭 g 路kg ~ (-1) 路kg ~ (-1) 路kg ~ (-1) 路kg ~ (-1). Conclusion under propofol anesthesia, the ED50 of remifentanil in combination with compound lidocaine cream is 0.91 渭 g / kg ~ 95% confidence interval of 0.80 渭 g / kg ~ (-1) 渭 g / kg ~ (-1) 路kg ~ (-1).
