【摘要】：目的:對比分析瑞芬太尼與芬太尼應(yīng)用于腹腔鏡手術(shù)麻醉中的麻醉效果和安全性。方法:選取86例在筆者所在醫(yī)院行腹腔鏡手術(shù)患者作為研究對象,對其臨床資料進(jìn)行回顧性分析。根據(jù)麻醉藥物將其分為A組(45例)和B組(41例),其分別應(yīng)用瑞芬太尼和芬太尼進(jìn)行麻醉誘導(dǎo)和維持。比較兩組患者血流動力學(xué)變化情況、麻醉效果以及不良反應(yīng)發(fā)生情況。結(jié)果:兩組患者誘導(dǎo)后1 min以及插管后1、2和5 min的HR和MAP與誘導(dǎo)前比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.05),而術(shù)后5 min則恢復(fù)至誘導(dǎo)前水平;且插管后1、2和5 min兩組患者HR和MAP比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);而A組患者誘導(dǎo)前后兩項(xiàng)指標(biāo)變化與B組相比更為穩(wěn)定。A組患者自主呼吸恢復(fù)時(shí)間、睜眼時(shí)間、拔管時(shí)間和離開恢復(fù)室時(shí)間均明顯短于B組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);A組呼吸道梗阻或屏氣及拔管后Sp O290%的發(fā)生率也明顯低于B組,但A組患者術(shù)后躁動發(fā)生率明顯高于B組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:瑞芬太尼應(yīng)用于腹腔鏡手術(shù)患者的麻醉中有助于患者維持更為平穩(wěn)的血流動力學(xué)變化,且患者術(shù)后蘇醒快,蘇醒質(zhì)量好,安全性高,具有較高的臨床應(yīng)用價(jià)值。
[Abstract]:Objective: To compare and analyze the anesthetic effect and safety of remifentanil and fentanyl used in laparoscopic anesthesia. Methods: a retrospective analysis was made in 86 cases of laparoscopic surgery in my hospital. The clinical data were analyzed retrospectively. According to narcotic drugs, they were divided into group A (45 cases) and group B (41 cases), which were applied respectively. Remifentanil and fentanyl were induced and maintained. Hemodynamic changes, anesthetic effects and adverse reactions were compared between the two groups. Results: the differences were statistically significant between the two groups of patients after induction of 1 min and HR and MAP of 1,2 and 5 min after intubation (P0.05), while 5 min recovered to induction after operation. The difference of HR and MAP in 1,2 and 5 min two groups after intubation was statistically significant (P0.05), while the changes of two indexes before and after induction in group A were more stable than those in the B group, and the time of eye opening, extubation time and leaving recovery room were significantly shorter than those in the group of B (P0.). The difference was statistically significant (P0.). 05): the incidence of Sp O290% after respiratory obstruction or breath holding and extubation in group A was also significantly lower than that in group B, but the incidence of postoperative restlessness in group A was significantly higher than that in group B (P0.05). Conclusion: the application of remifentanil to patients with laparoscopic surgery could help patients maintain more stable hemodynamic changes and patients. The postoperative recovery is fast, the quality of recovery is good, the safety is high, and it has high clinical application value.
