【摘要】：目的以剖腹產新生兒Apgar評分為基準,研究全身麻醉對新生兒的影響,探討能夠用于剖腹產的安全、有效、快捷的麻醉方法。方法計算機檢索中國學術期刊全文數據庫、維普中文科技期刊數據庫和萬方數據庫,收集全身麻醉用于剖腹產的回顧性病例對照研究,試驗組為全身麻醉組,對照組為椎管內神經阻滯麻醉組。檢索時限的截止日期為2015年10月12日。由2名研究人員按照納入和排除標準獨立篩選文獻、評價納入文獻的質量并提取資料,采用RevMan 5.3軟件對提取的數據進行統(tǒng)計學分析。結果納入32個回顧性病例對照研究,均為中文文獻,包括試驗組1 289例和對照組1 887例,研究對象均為ASA Ⅰ-Ⅱ級的孕產婦。meta分析結果顯示:(1)新生兒1 min Apgar評分影響的meta分析顯示,全身麻醉與椎管內神經阻滯麻醉的差別有統(tǒng)計學意義(MD=-0.18,P=0.009),椎管內神經阻滯麻醉對剖腹產新生兒1 min Apgar評分的影響更小;(2)新生兒5 min Apgar評分影響的meta分析顯示,全身麻醉與椎管內神經阻滯麻醉的差別無統(tǒng)計學意義(MD=0.00,P=1.00);(3)I-D時間的meta分析顯示,全身麻醉組的I-D時間較椎管內神經阻滯麻醉組短,全身麻醉具有明顯的優(yōu)勢(MD=-9.04,P0.001)。結論全身麻醉與椎管內神經阻滯麻醉對新生兒Apgar評分的影響的差異仍然不明確,但是全身麻醉用于剖腹產手術在娩出胎兒時間上有明顯的優(yōu)勢。
[Abstract]:Objective to study the effect of general anesthesia on newborn based on Apgar score of caesarean section and to explore a safe, effective and rapid anesthesia method for caesarean section. Methods the full text database of Chinese academic journals, the database of Chinese scientific and technological journals of Weipu and the database of Wanfang were searched by computer, and a retrospective case-control study of general anesthesia for caesarean section was collected. The experimental group was used as general anesthesia group. The control group was treated with intraspinal nerve block anesthesia. The deadline for retrieval is October 12, 2015. According to the inclusion and exclusion criteria, two researchers independently sifted the literature, evaluated the quality of the included literature and extracted the data, and analyzed the extracted data by RevMan 5.3 software. Results 32 retrospective case-control studies were conducted, all of which were Chinese literature, including 1 289 cases in the trial group and 1 887 cases in the control group.
