【摘要】：目的：觀察針刺剖宮產(chǎn)術(shù)后首次排氣、排便、腸鳴音恢復(fù)的時(shí)間及腹脹、腹痛、胃腸反應(yīng)等指標(biāo),探討足三里加上巨虛、足三里加曲池、足三里加尺澤對(duì)剖宮產(chǎn)術(shù)后胃腸功能恢復(fù)的有效性及差異性,為優(yōu)化針灸選穴提供依據(jù)。方法：按照納入標(biāo)準(zhǔn)納入行硬膜外麻醉的剖宮產(chǎn)術(shù)患者100例,隨機(jī)分為四組,足三里加上巨虛組、足三里加曲池組、足三里加尺澤組、對(duì)照組各25例。治療組予術(shù)后12h行針刺捻轉(zhuǎn)補(bǔ)法治療,每日兩次,中間間隔6h；對(duì)照組不予針刺干預(yù)。結(jié)果：1、首次排氣時(shí)間、排便時(shí)間、腸鳴音恢復(fù)正常時(shí)間,四組比較差異具有統(tǒng)計(jì)學(xué)意義,P0.05,以足三里加上巨虛組最佳。2、術(shù)后18h、24h腸鳴音分級(jí),四組比較差異具有統(tǒng)計(jì)學(xué)意義,P0.05,以足三里加上巨虛組最佳。術(shù)后36h腸鳴音分級(jí),四組比較差異具有統(tǒng)計(jì)學(xué)意義,P0.05。3、術(shù)后腹脹、胃腸反應(yīng)發(fā)生情況,四組比較差異具有統(tǒng)計(jì)學(xué)意義,P0.05,以足三里加上巨虛組最佳。4、術(shù)后36h、48h腹痛比較,P0.05,四組比較差異具有統(tǒng)計(jì)學(xué)意義,以足三里加上巨虛組最佳。結(jié)論：1、各治療組對(duì)促進(jìn)剖宮產(chǎn)術(shù)后胃腸功能恢復(fù)均有療效。2、足三里加上巨虛組優(yōu)于足三里加曲池組、足三里加尺澤組。3、足三里加曲池組與足三里加尺澤組無(wú)統(tǒng)計(jì)學(xué)差異。
[Abstract]:Objective: to observe the time of recovery of ventral distension, abdominal pain and gastrointestinal reaction after acupuncture for the first time after cesarean section, and to explore the indexes of Zusanli plus Juxu, Zusanli plus convoluted cistern, and other indexes, such as abdominal distension, abdominal pain, gastrointestinal reaction and so on. The effectiveness and difference of Zusanli plus zizawa in the recovery of gastrointestinal function after cesarean section provide basis for optimizing acupuncture and moxibustion. Methods: according to the inclusive criteria, 100 patients undergoing epidural anesthesia were randomly divided into four groups: Zusanli plus Juxu group, Zusanli plus Tuchi group, Zusanli plus zizawa group, and control group (25 cases each). The treatment group was treated with acupuncture twirling and toning at 12 hours after operation, twice a day, with an intermediate interval of 6 hours, while the control group was not treated with acupuncture intervention. Results: the first time of exhaust, the time of defecation, the time of bowel sound returning to normal were significantly different among the four groups (P 0.05). Zusanli plus Juxu group was the best. The difference among the four groups was statistically significant (P 0.05), and Zusanli plus Juxu group was the best. 36 h after operation, there were significant differences in the grade of bowel sounds among the four groups (P 0.05.3), abdominal distension and gastrointestinal reaction occurred after operation. The difference among the four groups was statistically significant (P 0.05). The best group was Zusanli plus Juxu group (P 0.05), and the abdominal pain was compared with P0.05 at 48h after operation. The difference among the four groups was statistically significant, especially in Zusanli plus Juxu group. Conclusion: 1, each treatment group has the curative effect to promote the recovery of gastrointestinal function after cesarean section. The effect of Zusanli plus Juxu group is better than that of Zusanli + Zizhi group, Zusanli plus zizawa group. There is no statistical difference between Zusanli plus zizawa group and Zusanli plus zizze group.
