【摘要】：目的:研究優(yōu)化胃鏡檢查術(shù)中針?biāo)帍?fù)合麻醉方案,利用正交設(shè)計(jì)方法篩選出最佳方案。方法:利用正交設(shè)計(jì)方法,選擇144例青中年胃鏡檢查患者隨機(jī)分為9組,分別采用針刺介入時(shí)機(jī)(A)、頭體腧穴選擇(B)、電針頻率參數(shù)(C)、麻醉藥物配伍(D)4個(gè)因素及相應(yīng)的3水平進(jìn)行干預(yù),觀察術(shù)中應(yīng)激反應(yīng)積分、異丙酚藥量、心率和血壓波動(dòng)值等指標(biāo),篩選出最佳組合方案。結(jié)果:影響療效的關(guān)鍵是麻醉藥物配伍,腧穴選擇次之,而針刺介入時(shí)機(jī)、電針頻率參數(shù)對(duì)療效的影響小。針?biāo)帍?fù)合麻醉的最佳方案為:麻醉藥物配伍選異丙酚+芬太尼(0.5μg/kg)、針刺腧穴選擇頭部和肢體穴位、針刺介入時(shí)機(jī)在術(shù)前10min進(jìn)行、電針頻率參數(shù)選擇低高頻交替(2/100Hz)或低頻(2Hz)。結(jié)論:青中年患者在胃鏡檢查術(shù)前10min進(jìn)行針刺頭部和肢體腧穴,加用電針頻率參數(shù)選擇低高頻交替(2/100Hz)或低頻(2Hz),并采用異丙酚+芬太尼(0.5μg/kg)麻醉藥物配伍的優(yōu)化組合方案能有效提高療效。
[Abstract]:Objective: to study the optimal anesthesia scheme of acupuncture and medicine during gastroscopy, and to select the best scheme by orthogonal design.Methods: 144 young and middle-aged patients were randomly divided into 9 groups by orthogonal design.Four factors and corresponding three levels of acupuncture intervention were used to observe the stress response score, propofol dosage, heart rate and blood pressure fluctuation, respectively.The best combination scheme is selected.Results: the key to the curative effect was the compatibility of anesthetic drugs, the selection of acupoints was the second, and the time of acupuncture intervention and the frequency parameter of electroacupuncture had little effect on the curative effect.The best scheme of combined acupuncture and medicine anesthesia was as follows: propofol fentanyl 0.5 渭 g / kg, acupuncture acupoints selected head and limb acupoints, acupuncture intervention time performed before 10min, electroacupuncture frequency parameters of low frequency alternating 2 / 100 Hz) or low frequency (2HzC) or low frequency (2HzN) of propofol fentanyl (0.5 渭 g / kg 路kg ~ (-1)).Conclusion: the head and limb acupoints were needled by 10min before gastroscopy in young and middle-aged patients.The optimal combination of electroacupuncture frequency parameters, low frequency alternating 2 / 100 Hz or low frequency 2 Hz, and propofol fentanyl 0.5 渭 g / kg was used to improve the efficacy of anaesthesia. The results showed that the optimal combination of propofol fentanyl and propofol fentanyl 0.5 渭 g / kg could improve the efficacy.
