【摘要】：目的對(duì)比觀察多孔前房維持器連接灌注液下與黏彈劑下行白內(nèi)障超聲乳化吸除聯(lián)合人工晶狀體植入手術(shù)(PHACO+IOL植入術(shù))后對(duì)眼表影響。方法納入分析的年齡相關(guān)性白內(nèi)障患者68例(68眼)隨機(jī)分成兩組。A組34例全程使用多孔前房維持器(anterior chamber maintainer,ACM)連接灌注液維持前房下行PHACO+IOL植入術(shù),B組34例常規(guī)黏彈劑維持前房下行PHACO+IOL植入術(shù)。觀察術(shù)前及術(shù)后1天、術(shù)后7天、術(shù)后30天的主觀干燥異物感、淚膜破裂時(shí)間(BUT)以及角膜熒光染色并積分(CFS)及無(wú)表面麻醉下淚液分泌實(shí)驗(yàn)(SⅠt)的改變。結(jié)果1.兩組內(nèi)患者各項(xiàng)指標(biāo)與術(shù)前相比,其中BUT在術(shù)后1天、7天、30天均較術(shù)前明顯縮短,CFS在術(shù)后1天、7天高于術(shù)前,SⅠt在術(shù)后1天明顯增加,術(shù)后7天降低。以上差異均具有統(tǒng)計(jì)學(xué)意義(P0.05)。2.A組術(shù)后1天的角膜熒光素染色并積分值改變量(3.47±0.71)比B組(2.94±0.78)多,A組術(shù)后1天的淚液分泌試驗(yàn)(SchirmerⅠ)改變量(4.50±1.60mm/5min)比B組(3.65±1.18 mm/5min)多,以上差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。而在術(shù)后7天和術(shù)后30天各項(xiàng)觀察指標(biāo)的改變量,兩組之間差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論與常規(guī)白內(nèi)障超聲乳化手術(shù)比較,灌注液下行白內(nèi)障超聲乳化吸除在術(shù)后早期會(huì)增加淚液分泌量,輕微加重角膜上皮損傷,但術(shù)后中晚期并未加重眼表組織的損害。
