【摘要】：研究背景男性乳房發(fā)育癥(Gynaecomastia, GYN)是指男性乳房的異常增大,常由各種因素導(dǎo)致的人體內(nèi)分泌失調(diào)繼而引起男性乳腺組織異常增生、乳房脂肪組織異常增生堆積的一種常見(jiàn)的疾病。這些因素可為生理性也可為病理性誘。男性乳腺發(fā)育癥患者近年來(lái)呈發(fā)展趨勢(shì),或與生活水平的提高,生活規(guī)律的變化有關(guān)。國(guó)外有學(xué)者報(bào)告該病的發(fā)病率占男性32%-65%。該病雖然為常見(jiàn)的良性疾病,但是由于患者及其家屬可明顯感覺(jué)其自身的形體異�；蛉毕�,給患者帶來(lái)較大的心理壓力和嚴(yán)重的精神負(fù)擔(dān),不少患者被發(fā)現(xiàn)正遭受抑郁癥、焦慮癥等心理問(wèn)題。劉晨、欒杰等人報(bào)告表明整形外科手術(shù)可以明顯的改善GYN患者的異常心理狀態(tài),糾正了患者消極的體像,提高患者自信心,恢復(fù)其正常的心理狀態(tài)。近年來(lái),越來(lái)越多的國(guó)內(nèi)外研究機(jī)構(gòu)對(duì)此疾病更為重視,該病的治療也取得了較大的進(jìn)展。 目的探討男性乳腺發(fā)育癥的臨床治療中小切口治療的可行性及其療效。為今后男性乳腺發(fā)育癥的臨床治療提供更為優(yōu)化的治療方式。 方法回顧分析山東大學(xué)齊魯醫(yī)院整形外科近一年收治的26例男性乳腺發(fā)育癥的病例,其中小切口治療為20例。手術(shù)均局部麻醉下進(jìn)行,經(jīng)乳暈周?chē)∏锌?2-3cm)切開(kāi)皮膚,沿乳腺表面及乳腺與胸大肌之間的間隙潛行分離腺體組織,切下乳腺組織送常規(guī)病理檢查。 結(jié)果全部20例手術(shù)均獲得成功,術(shù)中無(wú)并發(fā)癥、無(wú)輸血。平均時(shí)間約45.5分鐘(單側(cè)),術(shù)后全部均為甲級(jí)愈合,切口無(wú)感染,皮膚無(wú)壞死,皮下無(wú)血腫,乳暈感覺(jué)無(wú)障礙且適當(dāng)刺激可勃起,20例患者及其家屬對(duì)手術(shù)效果表示滿意。 結(jié)論乳暈小切口治療男性乳腺發(fā)育癥具有切口較為隱蔽、瘢痕不明顯、手術(shù)并發(fā)癥少、術(shù)后外形改善明顯、患者生活質(zhì)量提高。
[Abstract]:On the background of development of male breast (Gynaecomastia, GYN) refers to the abnormal increase of male breast, often by a variety of factors leading to endocrine disorders caused by abnormal hyperplasia and male breast tissue, a common disease of breast hyperplasia of adipose tissue accumulation. These factors can be physiologic or patho induced male breast. The development of patients with a development trend in recent years, with the improvement of living standards, the change law of life. Foreign scholars report the disease incidence rate for male 32%-65%. although the disease is a benign disease is common, but because of the patients and their families can obviously feel their own body abnormalities or defects, bring greater psychological the pressure and the serious mental burden to patients, many patients were found to suffer from depression, anxiety and other psychological problems. Liu Chen, Luan Jie et al report shows that plastic surgery can Obviously improving the abnormal state of mind of GYN patients, correcting the negative body image of patients, improving their self-confidence and restoring their normal mental state. In recent years, more and more domestic and foreign research institutions pay more attention to this disease, and the treatment of this disease has made great progress.
Objective to explore the feasibility and efficacy of small incision for the treatment of gynecomastia.
Methods a retrospective analysis of Qilu Hospital of Shandong University for nearly a year from 26 cases of gynecomastia cases, including 20 cases of small incision treatment. Were performed under local anesthesia, through small incision around the areola (2-3cm) skin incision along the gap between breast and breast surface stealth and pectoral muscle separation glands. Cut breast tissue sent for routine pathological examination.
Results all 20 cases were successfully operated, no intraoperative complications, no blood transfusion. The average time of about 45.5 minutes (one side), after the operation all healed, no infection, no skin necrosis or hematoma and areola without obstacle and appropriate stimulation of erection, 20 cases of patient and family satisfaction the results of surgery.
Conclusion the treatment of gynecomastia with areola small incision has a more concealed incision, less scar, fewer operative complications, and improved postoperative appearance, and the quality of life of patients is improved.
