【摘要】：目的總結(jié)應(yīng)用腹直肌肌皮瓣修復(fù)胸部慢性放射性潰瘍的經(jīng)驗(yàn)。方法 2010年以來(lái)收治胸部深度放射性潰瘍6例,全身麻醉下行胸壁放射性潰瘍創(chuàng)面清創(chuàng),切除壞死軟組織及變性肋骨,根據(jù)缺損大小及形狀,切取帶有腹壁上動(dòng)靜脈血管蒂的腹直肌肌皮瓣轉(zhuǎn)移修復(fù)胸壁缺損,重建胸壁。其中4例采用縱行腹直肌肌皮瓣;2例采用橫行腹直肌肌皮瓣修復(fù),供瓣區(qū)給予游離皮片移植或縫合修復(fù)。結(jié)果 1例患者皮瓣邊緣與創(chuàng)緣愈合不佳,給予擴(kuò)大切除局部病變皮膚范圍后縫合修復(fù)。其余皮瓣成活良好,創(chuàng)面修復(fù)滿意。術(shù)后隨訪1~3年,潰瘍無(wú)復(fù)發(fā),供瓣區(qū)無(wú)腹壁疝形成。結(jié)論清創(chuàng)后,應(yīng)用血運(yùn)豐富的腹直肌肌皮瓣覆蓋,可有效修復(fù)胸部慢性放射性潰瘍,是一種簡(jiǎn)單、安全、有效的治療方法。
[Abstract]:Objective to summarize the experience of rectus abdominis myocutaneous flap in repairing chronic radiation ulcer of chest.Methods six cases of chest deep radiation ulcer were treated in our hospital since 2010. The wound of chest wall radiation ulcer was debrided under general anesthesia. Necrotic soft tissue and degenerated ribs were removed according to the size and shape of the defect.Rectus abdominis musculocutaneous flap with superior abdominal arteriovenous pedicle was used to repair chest wall defect and reconstruct chest wall.Among them 4 cases were treated with longitudinal rectus abdominis myocutaneous flap 2 cases were repaired with transverse rectus abdominis myocutaneous flap and the donor region was repaired with free skin graft or suture.Results the edge and wound edge of skin flap were not well healed in one case.The remaining flaps survived well and the wounds were repaired satisfactorily.Conclusion after debridement, it is a simple, safe and effective method to repair chronic radiation ulcer of chest by using rectus abdominis myocutaneous flap with abundant blood circulation.
