【摘要】：目的探討全身麻醉與腰麻對(duì)老年患者行全髖關(guān)節(jié)置換術(shù)后早期認(rèn)知功能障礙(POCD)發(fā)生率的影響。方法選擇2015年2月至2016年2月84例在我院行擇期全髖關(guān)節(jié)置換術(shù)老年患者隨機(jī)分為全麻組與腰麻組。于術(shù)后第7天使用簡易精神狀態(tài)檢查(MMSE)量表評(píng)價(jià)認(rèn)知功能,記錄評(píng)分并使用“Z計(jì)分法”進(jìn)行POCD診斷。結(jié)果全麻組與腰麻組術(shù)后第7天POCD發(fā)生率分別為27.0%與17.9%,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。POCD獨(dú)立危險(xiǎn)因素為高齡(OR 1.250,95%CI 1.119-1.398)與低教育水平(OR 5.747,95%CI 1.261-26.185)。結(jié)論全身麻醉與腰麻對(duì)行全髖關(guān)節(jié)置換術(shù)老年患者術(shù)后早期POCD發(fā)生率的影響無顯著差異;高齡與低教育水平為POCD獨(dú)立危險(xiǎn)因素。
[Abstract]:Objective to investigate the effect of general anesthesia and spinal anesthesia on the incidence of early cognitive dysfunction (POCD) after total hip replacement in elderly patients. Methods 84 patients with elective total hip replacement were selected from February 2015 to February 2016 in our hospital. The patients were randomly divided into general anesthesia group and spinal anesthesia group. On the 7th day after operation, the cognitive function was evaluated with MMSE scale. Results the incidence of POCD in general anesthesia group and spinal anesthesia group was 27.0% and 17.9 respectively on the 7th day after operation. There was no significant difference in the independent risk factors of POCD: old age OR 1.25095 / CI 1.119-1.398) and low education level OR (P < 0.05). Conclusion there is no significant difference between general anesthesia and spinal anesthesia on the incidence of early POCD in elderly patients undergoing total hip arthroplasty. Old age and low level of education were independent risk factors for POCD.
