【摘要】：[目的]調(diào)查人工髖、膝關(guān)節(jié)置換術(shù)后嚴重內(nèi)科并發(fā)癥的特點和危險因素,為人工關(guān)節(jié)置換術(shù)的圍手術(shù)期管理和臨床決策提供參考。[方法]研究對象為2013年5月1日~12月31日期間行初次人工髖或膝關(guān)節(jié)置換術(shù)的患者。采集患者的基本信息、病史、麻醉、手術(shù)及術(shù)后并發(fā)癥情況,分析術(shù)后并發(fā)癥的特點和危險因素。[結(jié)果]共有1 050例患者行1 188個初次人工髖或膝關(guān)節(jié)置換術(shù)�；颊叩钠骄挲g為(59.9±11.9)歲,70.1%的患者為女性。人工關(guān)節(jié)置換術(shù)后嚴重內(nèi)科并發(fā)癥的發(fā)生率為4.8%,包括心梗(0.1%)、心絞痛(1.9%)、心律失常(1.2%)、心衰(0.4%)、腦梗(0.6%)、肺栓塞(0.2%)和慢性阻塞性肺病急性加重(0.4%)。90%的嚴重內(nèi)科并發(fā)癥發(fā)生于術(shù)后4 d內(nèi)。年齡≥70歲(OR:2.49,95%CI:1.17~5.29)和ASAⅢ級(OR:15.97,95%CI:2.66~95.79)是嚴重內(nèi)科并發(fā)癥的危險因素。[結(jié)論]人工髖、膝關(guān)節(jié)置換術(shù)后會出現(xiàn)嚴重的內(nèi)科并發(fā)癥。要重視患者的圍手術(shù)期管理,尤其是高齡和高ASA分級的患者。人工關(guān)節(jié)置換術(shù)后住院日不宜過短。
[Abstract]:[objective] to investigate the characteristics and risk factors of severe medical complications after hip and knee arthroplasty. To provide reference for perioperative management and clinical decision making of artificial joint replacement. [methods] the subjects of the study were the patients who underwent the first hip or knee arthroplasty from May 1st 2013 to December 31st. The basic information and history of the patients were collected. Anesthesia, surgery and postoperative complications, The characteristics and risk factors of postoperative complications were analyzed. [results] A total of 1 050 patients were treated with 1 188 primary hip or knee arthroplasty. The average age of the patients was 59.9 鹵11.9 years old. 70.1% of the patients were female. The incidence of severe internal complications was 4.8, including myocardial infarction 0.1, angina 1.9, arrhythmia 1.2, heart failure 0.4, cerebral infarction 0.6, pulmonary embolism 0.2) and chronic obstructive pulmonary disease acute exacerbation 0.40.90% of serious internal complications occurred within 4 days after operation.
