【摘要】：目的探討局部麻醉對(duì)老年腹股溝疝修補(bǔ)術(shù)患者免疫功能的影響。方法 70例行無(wú)張力疝修補(bǔ)術(shù)治療老年腹股溝疝患者隨機(jī)分為局部麻醉組和連續(xù)硬膜外麻醉組,每組各35例,分別采用局部麻醉法和連續(xù)硬膜外麻醉下施行腹股溝疝無(wú)張力修補(bǔ)術(shù),觀察兩組患者麻醉前1 d(T0)、術(shù)后1 d(T1)和術(shù)后3 d(T2)的白細(xì)胞(WBC)、淋巴細(xì)胞絕對(duì)值(LY)、白細(xì)胞介素6(IL-6)和腫瘤壞死因子α(TNF-α)水平。結(jié)果 T1時(shí)點(diǎn)WBC水平局部麻醉組明顯低于連續(xù)硬膜外麻醉組(P0.05),LY局部麻醉組明顯高于連續(xù)硬膜外麻醉組(P0.05),T2時(shí)點(diǎn)IL-6水平局部麻醉組明顯高于連續(xù)硬膜外麻醉組(P0.05)。結(jié)論局部麻醉對(duì)老年腹股溝疝修補(bǔ)術(shù)患者主要免疫功能指標(biāo)影響較小,可作為老年腹股溝疝無(wú)張力修補(bǔ)術(shù)的首選。
[Abstract]:Objective to investigate the effect of local anesthesia on the immune function of elderly patients undergoing inguinal hernia repair. Methods 70 cases of elderly patients with inguinal hernia treated by tension-free hernioplasty were randomly divided into local anesthesia group and continuous epidural anesthesia group with 35 cases in each group. Tension free repair of inguinal hernia was performed under local anesthesia and continuous epidural anesthesia, respectively. The levels of WBC, lymphocyte absolute value, interleukin-6 (IL-6) and tumor necrosis factor 偽 (TNF- 偽) were observed in the two groups at 1 day before anesthesia, 1 day after anesthesia, 1 day after operation and 3 days after operation. Results the level of WBC in local anesthesia group was significantly lower than that in continuous anesthesia group at T1 time point. The level of IL-6 in the epidural anesthesia group was significantly higher than that in the continuous epidural anesthesia group at the time point P0.05 / T 2. Conclusion the main immunological effects of local anesthesia on inguinal hernia repair in the elderly patients are significant higher than that in the continuous epidural anesthesia group. The influence of function index is small, It can be used as the first choice for tension-free repair of inguinal hernia in the elderly.
