活血散干預(yù)大鼠骨折模型早期骨愈合的實(shí)驗(yàn)研究

【摘要】:[目的]通過(guò)大鼠急性新鮮股骨骨折損傷模型的建立,運(yùn)用南京市中醫(yī)院院內(nèi)制劑活血散軟膏作為外敷治療骨折藥物,通過(guò)不同時(shí)間段測(cè)定用藥前后大鼠血液流變學(xué)、血漿粘度和血清堿性磷酸酶(ALP)、鈣(Ca)、磷(P)的變化,同時(shí)觀察組織病理學(xué)和X線掃描成像的改變,探討活血散促進(jìn)骨折愈合的機(jī)理,為臨床中藥外治方法提供了基礎(chǔ)理論依據(jù)。[方法]根據(jù)中藥藥理實(shí)驗(yàn)學(xué)實(shí)驗(yàn)方法,造模前一天,大鼠左側(cè)臀部及大腿后側(cè)用8%硫化鈉脫毛劑脫毛。用20%的烏拉坦溶液按體重(5ml/kg)行腹腔注射將大鼠淺麻醉,大鼠反抗能力減弱即為麻醉顯效;大鼠呈仰臥位,固定左側(cè)大腿呈外展內(nèi)旋的姿勢(shì),將造模支架固定于此并調(diào)節(jié)鐵碇,將在大腿中部用鈍骨刀加壓,助手提起500g的砝碼至30cm處,讓砝碼沿導(dǎo)引桿以自由落體的方式與鋼板撞擊,力傳導(dǎo)至鈍骨刀片上,撞擊股骨中段造成骨折。造模后立即拍攝患肢正位X線片,了解骨折類型及移位程度;要求骨折類型為橫斷或短斜型,顯示造模成功。打擊粉碎性骨折及皮膚破損的大鼠予以剔除。選取造模成功大鼠36只隨機(jī)分為兩大組:A組對(duì)照組18只,外涂凡士林/羊毛脂2:1混合膏5g加特殊夾板固定,根據(jù)取材時(shí)間再隨機(jī)分成a,b,c3組(給藥后第10,20,30d);B組實(shí)驗(yàn)組18只,與骨折處外涂活血散軟膏5g加特殊夾板固定,根據(jù)取材時(shí)間再隨機(jī)分成a,b,c3組(給藥后第10,20,30d)。造模成功1小時(shí)后實(shí)施相關(guān)治療處理。在用藥后的第10天、20天、30天,各組任取大鼠6只,麻醉后采集股動(dòng)脈血進(jìn)行血液流變學(xué)、血漿粘度及血生化測(cè)定,取血結(jié)束后采用牽拉使頸椎脫臼的方法快速處死大鼠,然后解剖分離骨折段股骨,取股骨全長(zhǎng)標(biāo)本進(jìn)行X線掃描成像、骨痂病理組織形態(tài)學(xué)檢查。[結(jié)果]活血散膏組在對(duì)骨折模型大鼠血漿粘度、血漿粘度、血清ALP、Ca、P含量的影響,以及骨折模型大鼠骨痂組織形態(tài)學(xué)、骨折模型大鼠股骨X線片的影響均優(yōu)于模型組;钛⒃谥委20、30天時(shí),可以顯著改善骨折模型大鼠的血液流變學(xué),降低全血ls-l、5s-l、30s-1的切變率,可以顯著降低骨折模型大鼠的血漿粘度,可以顯著升高骨折模型大鼠血清ALP、Ca、P含量。在不同時(shí)間節(jié)點(diǎn)上活血散組的骨折斷端病理切片均可見軟骨性骨痂、小梁狀骨的產(chǎn)生明顯優(yōu)于對(duì)照組,X線片上顯示的骨痂生長(zhǎng)影也明顯優(yōu)于對(duì)照組。[結(jié)論]活血散局部外用對(duì)大鼠骨折模型早期骨愈合有明顯促進(jìn)作用,其作用機(jī)理可能與改善血循環(huán)和促進(jìn)成骨代謝相關(guān)。
[Abstract]:[Objective] to establish a model of acute fresh femoral fracture injury in rats and to use Huoxue Powder ointment as an external application drug in Nanjing Hospital of traditional Chinese Medicine. The changes of hemorheology, plasma viscosity and serum alkaline phosphatase (ALPX), calcium and calcium (Cam) in rats before and after treatment were measured at different time periods. At the same time, histopathology and X-ray scanning imaging were observed, and the mechanism of promoting fracture healing by Huoxue Powder was discussed, which provided the basic theoretical basis for the external treatment of traditional Chinese medicine. [Methods] according to the experimental method of pharmacology of traditional Chinese medicine, the model was made one day before. The rats left buttocks and thighs were treated with 8% sodium sulphide hair remover. The rats were anesthetized by intraperitoneal injection with 20%% uratan solution (5 ml / kg). The decrease of resistance ability of rats is the obvious effect of anesthesia. The rats presented supine position, fixed the left thigh in the position of abduction and internal rotation, fixed the scaffold and adjusted the iron Anchorage, and pressed it with a blunt bone knife in the middle of the thigh, and the assistant lifted 500g weight to 30cm. Let the weight strike the steel plate along the guide rod in the way of free falling body, and the force is transmitted to the blunt bone blade, and the fracture is caused by hitting the middle part of the femur. Immediately after making the model, the orthostatic X-ray film of the affected limb is taken to understand the type of fracture and the degree of displacement. The fracture type was required to be transverse or short oblique type, which showed that the model was successful. The rats that hit the comminuted fracture and the skin damaged were eliminated. Thirty-six successful rats were randomly divided into two groups: group A, group A, control group, n = 18. The Vaseline / lanolin 2: 1 mixed cream was fixed with special splint at 5g. According to the time of extraction, it was randomly divided into 3 groups (10 ~ 20 ~ 30 days after administration). In group B, 18 rats were treated with Huoxue Powder ointment at the fracture site and fixed with special splint. According to the time of extraction, they were randomly divided into 3 groups (1020). 30 days after the successful establishment of the model 1 hour after the implementation of the relevant treatment. 10 days after the use of drugs 20 days to 30 days, each group of 6 rats, the femoral artery blood was collected after anesthesia for hemorheology. The plasma viscosity and blood biochemistry were measured. The rats were killed quickly by pulling the cervical vertebrae dislocated after the blood was taken. Then the femur was dissected and the whole femur specimens were taken for X-ray imaging. Histopathological examination of callus. [Results: the effect of Huoxue Powder ointment on plasma viscosity, plasma viscosity and the content of serum ALP in rats with fracture, and the histomorphology of callus in fracture model rats. The effect of X-ray film of femur in fracture model rats was better than that in model group. The hemorheology of fracture model rats could be significantly improved and the whole blood ls-ln 5s-l could be reduced by Huoxue Powder after 20 ~ 30 days of treatment. The shear rate of 30 s ~ (-1) could significantly decrease the plasma viscosity and increase the serum ALP ~ (+) Ca in the fracture model rats. P content. Cartilage callus could be seen in pathological sections of fracture broken end in Huoxue Powder group at different time points. Trabecular bone formation was obviously superior to the growth shadow of bone callus on X ray film of control group. [Conclusion: the local application of Huoxue Powder can obviously promote bone healing in the early stage of fracture model in rats, and its mechanism may be related to improving blood circulation and promoting osteogenesis metabolism.
