【摘要】：目的探討新建立的澳門(mén)健康成人脈沖振蕩肺功能各主要指標(biāo)預(yù)計(jì)值方程式在老年COPD患者中的診斷價(jià)值。方法對(duì)97例老年COPD患者及47例健康老年人應(yīng)用脈沖振蕩肺功能測(cè)定氣道阻力,與常規(guī)肺通氣功能、新建立的澳門(mén)健康成人脈沖振蕩肺功能各主要指標(biāo)預(yù)計(jì)值和廠家預(yù)計(jì)值進(jìn)行比較,對(duì)比分析它們對(duì)COPD的診斷價(jià)值。結(jié)果 COPD男女兩組Zrs、R5、(R5-R20)、RF均高于健康對(duì)照組,男性COPD組R20亦高于健康對(duì)照組;X5、X20 COPD組均低于健康對(duì)照組。除女性組R5 A/P1(廠家預(yù)計(jì)值)與FEV1/FVC%、FEV1/Pre%相關(guān)性稍高外,余男女兩組Zrs、R20、X5等指標(biāo)均為P2(新建立的澳門(mén)健康成人脈沖振蕩肺功能各主要指標(biāo)預(yù)計(jì)值)與FEV1/FVC%、FEV1/Pre%相關(guān)性最高。RF、Zrs、(R5-R20)、R5、R20、X5(P2-A)等指標(biāo)均與FEV1/FVC%、FEV1/Pre%呈顯著負(fù)相關(guān),其中RF的相關(guān)性最為密切,其余依次為女性組:Zrs(A/P2)、(R5-R20)、X5(P2-A)、R5、R20;男性組,與FEV1/FVC%相關(guān)的順序?yàn)閆rs(A/P2)、X5(P2-A)、(R5-R20)、R5、R20。與FEV1/Pre%相關(guān)的順序?yàn)閄5(P2-A)、Zrs(A/P2)、(R5-R20)、R5、R20。IOS各指標(biāo)ROC曲線下面積分析顯示,Zrs(A/P2)、R5(A/P2)、R20(A/P2)、R20(A/P2)、X5(P2-A)分別大于Zrs(A/P1)及Zrs(A)、R5(A/P1)、R20(A/P1)、R20(A/P1)、X5(P1-A)。男性組Zrs(A/P2)%≥113.7時(shí),R5(A/P2)%≥133.4時(shí),R20(A/P2)%≥77.4時(shí),X5(P2-A)≥0.035時(shí),RF≥15.9時(shí),各約登指數(shù)最高。女性組Zrs(A/P2)%≥127.7時(shí),R5(A/P2)%≥121.7時(shí),R20(A/P2)%≥129.5時(shí),X5(P2-A)≥0.005時(shí),RF≥18時(shí),各約登指數(shù)最高。結(jié)論脈沖振蕩肺功能對(duì)COPD具有診斷價(jià)值,根據(jù)澳門(mén)本地人群建立的正常預(yù)計(jì)值方程式在診斷COPD時(shí),其敏感性、特異性均高于廠家預(yù)計(jì)值,建議推廣應(yīng)用。
[Abstract]:Objective to investigate the diagnostic value of the predicted values of pulse oscillatory pulmonary function in elderly patients with COPD. Methods Airway resistance was measured by pulse oscillatory pulmonary function in 97 elderly patients with COPD and 47 healthy elderly. The predicted values of main indexes and manufacturers of pulse oscillatory pulmonary function in healthy adults in Macao were compared with those of conventional pulmonary ventilation, and their diagnostic value for COPD was compared and analyzed. Results Zrs,R5, (R5-R20), RF) in both male and female groups of COPD was higher than that in healthy control group, R20 in male COPD group was also higher than that in healthy control group, and X5, X20 COPD group was lower than that in healthy control group. Except that the correlation between R5 A/P1 and FEV1/FVC%,FEV1/Pre% was slightly higher in the female group, the indexes such as Zrs,R20,X5 were P2 (the predicted values of the main indexes of pulse oscillatory lung function in healthy adults in Macao) were the highest correlation with FEV1/FVC%,FEV1/Pre%.
