合并骨侵蝕及間質性肺病的類風濕關節(jié)炎患者血清IL-25水平升高

【摘要】:目的分析類風濕關節(jié)炎(RA)患者疾病發(fā)展進程中血清白細胞介素25(IL-25)和IL-17水平以及與骨侵蝕及合并間質性肺病(ILD)的關系。方法 ELISA檢測117例RA患者和56例健康對照血清IL-25和IL-17水平;檢測血沉(ESR)、C反應蛋白(CRP)、類風濕因子(RF)、抗環(huán)瓜氨酸(CCP)抗體水平;胸部高分辨率CT(HR-CT)診斷ILD;肌骨超聲檢查判斷骨侵蝕情況;Sharp評分劃分影像學等級;VAS評分和28個關節(jié)疾病活動度評分(DAS28)反映RA患者的疾病活動情況。比較各組之間IL-25和IL-17的水平差異并分析相關性。結果 RA組的IL-25和IL-17水平明顯高于健康對照組,有骨侵蝕的RA患者IL-25水平顯著高于無骨侵蝕的RA患者。有ILD的RA患者IL-25水平顯著高于不伴有ILD的RA患者。IL-25水平與RF-IgG、RF-IgA和RF-IgM均呈正相關(r=0.285、0.314、0.380),同時,IL-17水平與RF-IgG和RF-IgM呈正相關(r=0.198、0.273),但二者與抗CCP抗體之間無相關性。結論合并骨侵蝕及間質性肺病的類風濕關節(jié)炎患者血清IL-25水平升高。
[Abstract]:Objective to analyze the relationship between serum IL-25 (IL-25) and IL-17 levels and bone erosion and (ILD) in patients with rheumatoid arthritis (RA). Methods ELISA was used to detect serum IL-25 and IL-17 levels in 117 patients with RA and 56 healthy controls, erythrocyte sedimentation rate (ESR), C reactive protein (CRP), rheumatoid factor (RF), anti-cyclic citrulline (CCP) antibody level, chest high resolution CT (HR-CT) to diagnose ILD; muscle bone ultrasound to judge bone erosion, Sharp score to classify imaging grade. VAS score and 28 joint disease activity score (DAS28) reflected the disease activity of RA patients. The differences of IL-25 and IL-17 levels among the three groups were compared and the correlation was analyzed. Results the levels of IL-25 and IL-17 in RA group were significantly higher than those in healthy control group, and the IL-25 levels in RA patients with bone erosion were significantly higher than those in RA patients without bone erosion.
