【摘要】：目的探討臨床藥師參與重癥肺炎患者藥學(xué)監(jiān)護(hù)的方法和作用,促進(jìn)醫(yī)院抗菌藥物的合理應(yīng)用。方法以2014年1月1例泛耐藥鮑氏不動(dòng)桿菌重癥肺炎患者治療為例,臨床藥師從制定個(gè)體化用藥方案、提供用藥依據(jù)、監(jiān)測(cè)藥物不良反應(yīng)等,提供全程的藥學(xué)監(jiān)護(hù),應(yīng)用超常規(guī)劑量的替加環(huán)素治療,并進(jìn)行臨床效果評(píng)價(jià)。結(jié)果臨床藥師積極參與藥物治療過程,對(duì)患者實(shí)施合理的藥學(xué)監(jiān)護(hù),患者體溫、血常規(guī)、C-反應(yīng)蛋白等顯著好轉(zhuǎn),兩下肺病灶較前顯著吸收,痰培養(yǎng)鮑氏不動(dòng)桿菌為陰性,取得了滿意的治療效果。結(jié)論對(duì)重癥肺炎患者開展藥學(xué)監(jiān)護(hù)非常必要,保證了患者治療的安全性和有效性,有利于臨床合理用藥及醫(yī)院整體用藥水平的提高。
[Abstract]:Objective To study the methods and effects of clinical pharmacists in patients with severe pneumonia and to promote the rational application of antibiotics in hospital. Methods By taking the treatment of 1 January 2014 as an example, the clinical pharmacists provide the whole-course pharmaceutical monitoring from the development of the individualized drug administration scheme, provide the basis of the medication, monitor the adverse reaction of the drug, etc., and apply the supernormal dose of tigecycline for the treatment. And the clinical effect evaluation is carried out. Results The clinical pharmacists were actively involved in the drug treatment, and the patients were given reasonable pharmaceutical monitoring, the temperature of the patient, the blood routine, the C-reactive protein and the like improved significantly, and the lung lesions were significantly absorbed before the lung lesions, and the sputum culture of the Acinetobacter baumannii was negative, and the satisfactory therapeutic effect was obtained. Conclusion It is necessary to carry out the pharmaceutical care in the patients with severe pneumonia, and the safety and effectiveness of the patient's treatment are ensured, which is beneficial to the clinical rational use and the improvement of the overall drug level of the hospital.
