【摘要】：目的分析原發(fā)性自發(fā)性氣胸(PSP)MSCT胸部影像特征,探討其形成機(jī)制。方法搜集PSP 45例及性別、年齡匹配胸部無明確病變(對照組)和繼發(fā)性自發(fā)性氣胸(SSP)患者各45例。測量前兩組第六肋骨(6~(th))傾斜度、氣管分叉平面胸廓內(nèi)徑,觀察肋骨內(nèi)突(PRIL)、肺氣腫樣改變(BB)及氣胸組胸膜孔情況。組間變量值和計(jì)量數(shù)差異分別采用t和χ~2檢驗(yàn)。結(jié)果 PSP組與對照組6~(th)傾斜度分別為:(54.5±16.8)°、(65.6±17.5)°(t=6.79,P0.05);10~19歲、20~29歲和30~39歲氣管分叉平面胸廓橫徑/前后徑值分別為:2.65±1.24、2.58±1.02、2.61±1.12和2.04±0.74、2.08±0.87、2.05±0.78(t=4.28、4.05、4.39,P值均0.05);PRIL、BB檢出例數(shù)分別為:25和10例(χ~2=8.16,P0.05);35和4例(χ~2=40.72,P0.01)。PSP和SSP胸膜孔檢出率分別為26.67%和6.67%(χ~2=5.12,P0.01)。結(jié)論胸廓扁平、PRIL、BB及胸膜孔為PSP特征影像,胸廓扁平增加跨肺壓利于BB形成,PRIL增加胸膜壓迫磨損,易致BB破裂或胸膜薄弱漏氣形成PSP及胸膜孔。
[Abstract]:Objective to analyze the chest imaging features of primary spontaneous pneumothorax (PSP) MSCT) and to explore its formation mechanism. Methods 45 cases of PSP, 45 cases of sex, 45 cases of age matched chest without definite lesions (control group) and 45 cases of secondary spontaneous pneumothorax (SSP) were collected. The inclination of the sixth ribs (6 ~ (th) and the inner diameter of the chest of the trachea bifurcation plane were measured in the first two groups. The changes of (BB) in the pulmonary emphysema of the internal process of the ribs and the pleural foramen in the pneumothorax group were observed. T and Chi ~ 2 tests were used to test the value of variables and the number of measures among groups, respectively.
