【摘要】：目的探討急診重癥監(jiān)護病房(EICU)呼吸機相關性肺炎(VAP)患者中鮑氏不動桿菌感染的臨床預測因素,以期提高預防意識,降低其感染發(fā)生率。方法回顧性分析2009年7月-2015年6月243例入住EICU的VAP患者臨床資料,數(shù)據(jù)采用SPSS 18.0軟件進行統(tǒng)計分析。結果 243例VAP患者發(fā)生鮑氏不動桿菌性呼吸機相關性肺炎(AB-VAP)54例,發(fā)生率為22.22%;單因素分析發(fā)現(xiàn),多種因素有預測AB-VAP價值;多因素logistic回歸分析發(fā)現(xiàn),機械通氣時間≥15d、急性生理及慢性健康評分≥21、糖皮質激素的使用及氣管切開具有獨立預測價值。結論長時間機械通氣、急性生理及慢性健康Ⅱ評分高、使用激素及氣管切開為EICU VAP患者發(fā)生鮑氏不動桿菌感染的獨立預測因素,盡早脫機、加強營養(yǎng)支持治療、謹慎使用激素、嚴把氣管切開指征等措施有可能降低AB-VAP的發(fā)生。
[Abstract]:Objective to investigate the clinical predictors of Acinetobacter baumannii infection in (VAP) patients with (EICU) ventilator-associated pneumonia in emergency intensive care unit (ICU) in order to improve the awareness of prevention and reduce the incidence of Acinetobacter baumannii infection. Methods the clinical data of 24 VAP patients admitted to EICU from July 2009 to June 2015 were analyzed retrospectively. the data were statistically analyzed by SPSS 18.0 software. Results 54 patients (22.22%) developed Acinetobacter baumannii ventilator-associated pneumonia (AB-VAP) in 24 VAP patients, and univariate analysis showed that many factors were valuable in predicting AB-VAP.
