【摘要】：目的探究組合吸痰管結合洗必泰對ICU應用呼吸機患者醫(yī)院感染的防控價值。方法選擇2014年10月-2016年10月醫(yī)院ICU應用呼吸機住院患者98例,隨機分為對照組和試驗組,各49例;對照組給予常規(guī)棉球擦拭處理,試驗組給予組合式吸痰管結合洗必泰處理,評價兩組患者的口腔評分(OAG)、口腔干預效果及肺部感染評分(CPIS)等。結果治療后,對照組的OAG評分(18.73±7.96)高于試驗組(15.34±5.28)(P0.05);試驗組發(fā)生輕度口腔異味者、2級以上口腔異味者及口腔殘留物檢出率,口腔陽性菌檢出、牙菌斑指數及口腔黏膜損傷指標均低于對照組(P0.05);對照組CPIS評分顯著低于試驗組(P0.05);對照組的血液學指標高于試驗組(P0.05)。結論組合吸痰管結合洗必泰對ICU應用呼吸機患者醫(yī)院感染具有一定的防控價值,能夠降低住院患者口腔細菌感染率、肺部感染率,合理提高口腔黏膜的防御能力,降低住院時間,一定程度上提高患者的生活質量。
[Abstract]:Objective to explore the value of combined sputum suction tube combined with chlorhexidine in the prevention and control of nosocomial infection in ICU patients with ventilator. Methods from October 2014 to October 2016, 98 patients with ICU were randomly divided into control group and experimental group with 49 cases in each group. The control group was treated with routine cotton ball wiping, the experimental group was treated with combined sputum suction tube combined with chlorhexidine. The oral intervention effect of (OAG), and pulmonary infection score (CPIS) were evaluated in both groups. Results after treatment, the OAG score of the control group (18.73 鹵7.96) was higher than that of the experimental group (15.34 鹵5.28) (P0.05). In the test group, the detection rate of the patients with mild odour, the detection rate of oral residue, the plaque index and the index of oral mucosal injury were lower than those of the control group (P0.05). The CPIS score of the control group was significantly lower than that of the experimental group (P0.05), and the hematological index of the control group was higher than that of the experimental group (P0.05). Conclusion combination of sputum suction tube and chlorhexidine can prevent and control nosocomial infection in ICU patients with ventilator, reduce oral bacterial infection rate and lung infection rate, and reasonably improve oral mucosal defense ability. Reduce the length of stay, to a certain extent improve the quality of life of patients.
