【摘要】：目的分析急性有機(jī)磷中毒合并呼吸衰竭患者發(fā)生醫(yī)院感染的相關(guān)因素,提出相應(yīng)的護(hù)理對策,以降低感染率。方法回顧性分析2011年6月-2013年6月81例急性有機(jī)磷中毒合并呼吸衰竭患者的臨床資料,對其中26例發(fā)生醫(yī)院感染患者各相關(guān)因素進(jìn)行對比分析,并提出相應(yīng)的護(hù)理對策。結(jié)果急性有機(jī)磷中毒合并呼吸衰竭患者醫(yī)院感染部位分布以呼吸道為主,占57.69%,其次為胃腸道和泌尿系統(tǒng),各占15.38%,其他感染占11.55%;單因素分析結(jié)果顯示,年齡60歲、住院時間14d、氣管插管、氣管切開、意識狀態(tài)昏迷、多種抗菌藥物聯(lián)用、侵入性操作是急性有機(jī)磷中毒合并呼吸衰竭醫(yī)院感染的相關(guān)因素(P0.05),多因素logistic回歸分析結(jié)果顯示,年齡大、住院時間長、氣管插管、侵入性操作、昏迷是感染的高危因素(P0.05)。結(jié)論急性有機(jī)磷中毒合并呼吸衰竭醫(yī)院感染的相關(guān)因素較多,在護(hù)理過程中應(yīng)加強(qiáng)環(huán)境的無菌控制,嚴(yán)格執(zhí)行無菌操作,做好基礎(chǔ)護(hù)理等,減少侵入性操作,合理使用抗菌藥物,以控制和降低醫(yī)院感染率。
[Abstract]:Objective to analyze the related factors of nosocomial infection in patients with acute organophosphorus poisoning and respiratory failure, and to put forward corresponding nursing measures to reduce the infection rate. Methods the clinical data of 81 patients with acute organophosphorus poisoning complicated with respiratory failure from June 2011 to June 2013 were analyzed retrospectively. The relative factors of 26 patients with nosocomial infection were compared and the corresponding nursing countermeasures were put forward. Results the distribution of nosocomial infection in patients with acute organophosphorus poisoning complicated with respiratory failure was mainly respiratory tract (57.69), gastrointestinal tract and urinary system (15.38) and other infections (11.55). Univariate analysis showed that the age of 60 years, hospital stay for 14 days, tracheal intubation, tracheotomy, consciousness coma, multiple antibiotics combined, Invasive operation was the related factor of nosocomial infection of acute organophosphorus poisoning complicated with respiratory failure (P0.05). Multivariate logistic regression analysis showed that age, length of stay, tracheal intubation, invasive operation, and so on. Coma was a high risk factor for infection (P0.05). Conclusion there are many related factors in acute organophosphorus poisoning complicated with respiratory failure nosocomial infection. In the nursing process, we should strengthen the aseptic control of the environment, strictly carry out the aseptic operation, do well the basic nursing, and reduce the invasive operation. Rational use of antimicrobial agents in order to control and reduce the nosocomial infection rate.
