生命支持下改良洗胃術搶救有機磷農藥中毒呼吸衰竭研究

【摘要】:目的在臨床工作中認真總結經驗,尋找最佳洗胃方法,提高洗胃質量,給重度有機磷農藥中毒患者帶來生機。方法將49例急性有機磷農藥中毒致呼吸衰竭的患者分為兩組,對照組(23例)進行傳統洗胃,觀察組(26例)采取改良洗胃術,迅速給予氣管插管、呼吸機輔助呼吸,維持生命體征。地塞米松迅速消除咽部水腫,經鼻插入胃管進行間斷降梯式反復洗胃,留置胃管持續(xù)負壓引流、堿化溶解等措施。結果觀察組采用改良洗胃術,反復插胃管率降低,能迅速達阿托品化,對胃黏膜的損傷小,預后良好,與對照組傳統的洗胃方法比較,差異有統計學意義(P0.05)。結論生命支持下改良洗胃術搶救有機磷農藥中毒呼吸衰竭患者,洗胃徹底,不良反應小,預后良好,方法優(yōu)于傳統洗胃術,搶救成功率大大提高,病死率逐漸降低。
[Abstract]:Objective to summarize the experience in clinical work, to find the best method of gastric lavage, to improve the quality of gastric lavage, and to bring life to the patients with severe organophosphorus pesticide poisoning. Methods Forty-nine patients with respiratory failure caused by acute organophosphorus pesticide poisoning were divided into two groups: the control group (23 cases) received traditional gastric lavage, the observation group (26 cases) received modified gastric lavage, tracheal intubation and ventilator assisted respiration. Maintain vital signs. Dexamethasone quickly eliminated the edema of pharynx and inserted gastric tube through nose to wash stomach repeatedly with intermittent descending ladder. Results in the observation group, the rate of gastric tube was decreased by modified gastric lavage, the rate of gastric tube was decreased, and atropine was rapidly achieved. The gastric mucosa was damaged little and the prognosis was good. The difference was statistically significant compared with the traditional gastric lavage method in the control group (P0.05). Conclusion the modified gastric lavage under the support of life to rescue the patients with respiratory failure caused by organophosphorus pesticide poisoning has the advantages of thorough gastric lavage, small adverse reaction and good prognosis. The method is superior to the traditional gastric lavage, and the success rate of rescue is greatly improved, and the mortality rate is gradually reduced.
