【摘要】：目的：通過制備兔急性肺血栓栓塞癥(APTE)模型,(1)觀察APTE后TNF-α、IL-4和工L-1β的水平的變化。(2)給予外源性瘦素干預(yù),觀察外源性瘦素對兔APTE后TNF-a、IL-1β及超敏C反應(yīng)蛋白(Hs-CRP)水平的影響,探討炎癥因子與肺損傷的關(guān)系以及如何調(diào)控對肺損傷起到保護(hù)作用。方法：選取健康新西蘭兔30只,隨機(jī)分為三組：對照組(同樣行頸內(nèi)靜脈穿刺),模型組分生理鹽水(NS)對照組及瘦素預(yù)處理組。用ELISA方法檢測血清,術(shù)畢行病理分析。結(jié)果：1、栓塞后工L-4在發(fā)病早期未見升高,而在發(fā)病后6h明顯升高,TNF-α及IL-1β水平明顯高于對照組。2、形態(tài)學(xué)觀察,栓塞后肺動脈內(nèi)血栓形成,組織萎縮、出血及炎性反應(yīng)明顯。3、瘦素預(yù)處理組,TNF-α、IL-1β、Hs-CRP水平明顯下降；生理鹽水模型對照組明顯升高于健康對照組。4、病理所見瘦素干預(yù)組缺血滲出明顯輕于模型對照組。結(jié)論：1、急性肺血栓栓塞癥后炎癥因子發(fā)生的不同變化,在引起急性肺損傷中起重要作用。2、瘦素通過對急性肺血栓栓塞癥后炎性因子TNF-α、IL-1β和Hs-CRP的水平調(diào)控,從而對肺損傷起到了保護(hù)作用。
[Abstract]:Objective: to establish (APTE) model of acute pulmonary thromboembolism in rabbits, (1) to observe the changes of TNF- 偽, IL-4 and L -1 尾 after APTE. (2) to observe the effect of exogenous leptin on TNF-a, after APTE. The effects of IL-1 尾 and high sensitive C-reactive protein (Hs-CRP) on the levels of inflammatory factors and lung injury were studied. Methods: thirty healthy New Zealand rabbits were randomly divided into three groups: control group (same as internal jugular vein puncture), model group of normal saline (NS) control group and leptin pretreatment group. ELISA method was used to detect serum and pathological analysis was performed. Results: 1. After embolization, L-4 did not increase at the early stage of embolism, but increased significantly at 6 h after embolization. The levels of TNF- 偽 and IL-1 尾 were significantly higher than those in the control group. The levels of TNF- 偽, IL-1 尾 and Hs-CRP were significantly decreased in leptin preconditioning group. The level of ischemia exudation in the model control group was significantly lower than that in the model control group, and that in the leptin intervention group was significantly less than that in the model control group. Conclusion: 1. The different changes of inflammatory factors after acute pulmonary thromboembolism play an important role in acute lung injury. 2. Leptin plays an important role in the treatment of inflammatory factor TNF- 偽 after acute pulmonary thromboembolism. The level of IL-1 尾 and Hs-CRP is regulated, which protects against lung injury.
