【摘要】：目的探討徑向超聲(EBUS)聯(lián)合虛擬導(dǎo)航(VBN)對肺外周結(jié)節(jié)的診斷價值。方法納入2014年7月至2015年7月就診唐都醫(yī)院呼吸科的肺外周結(jié)節(jié)病變患者240例(0.8cm直徑3cm),根據(jù)引導(dǎo)設(shè)備不同,分為徑向超聲聯(lián)合虛擬導(dǎo)航組(EBUS+VBN組)、徑向超聲組(EBUS組)、虛擬導(dǎo)航組(VBN組)和對照組,分別行4組間診斷率的比較及病變不同大小時診斷率的比較,EBUS+VBN組與EBUS組定位病變時間及操作時間的比較。結(jié)果 240例患者中,EBUS+VBN組診斷率最高(81.67%),4組間診斷率差異有統(tǒng)計學(xué)意義(χ~2=19.34,P=0.00);EBUS+VBN組、EBUS組在直徑小于或等于2cm的病灶中診斷率低于直徑大于2cm的病灶,但差異無統(tǒng)計學(xué)意義(χ~2=2.04、3.40,P=0.15、0.07);EBUS+VBN組較EBUS組定位病變時間及操作時間均短,且差異無統(tǒng)計學(xué)意義(P=0.03、0.04)。結(jié)論徑向超聲聯(lián)合虛擬導(dǎo)航可提高肺外周結(jié)節(jié)的診斷率,縮短定位病變時間及操作時間。
[Abstract]:Objective to evaluate the diagnostic value of radial ultrasound (EBUS) combined with virtual navigation (VBN) in peripheral pulmonary nodules. Methods from July 2014 to July 2015, 240 patients with peripheral pulmonary nodule lesions (0.8cm diameter 3cm) were enrolled in Department of Respiratory, Tangdu Hospital. They were divided into three groups: radial ultrasound combined with virtual navigation group (EBUS VBN group), radial ultrasound group (EBUS group), virtual navigation group (VBN group) and control group. The diagnostic rate of, EBUS VBN group and EBUS group were compared respectively. Results the diagnostic rate of, EBUS VBN group was the highest (81.67%). There was significant difference among the four groups (蠂 ~ 2 ~ (19.34) P ~ (0.00); EBUS VBN group). The diagnostic rate in EBUS group was lower than that in the lesion whose diameter was less than or equal to that of 2cm. But the difference was not statistically significant (蠂 ~ 2 ~ 2. 04 ~ 3. 40 ~ 3. 40); EBUS VBN group vs EBUS group). The lesion time and operation time of EBUS group were shorter than that of EBUS group, and the difference was not statistically significant (P0. 03 ~ 0. 04). Conclusion Radial ultrasound combined with virtual navigation can improve the diagnosis rate of peripheral pulmonary nodules and shorten the time of localization and operation.
