【摘要】：背景與目的肺癌是慢性阻塞性肺疾病(慢阻肺)重要的合并癥,會顯著影響慢阻肺患者的預(yù)后。慢阻肺也會影響肺癌患者的術(shù)后并發(fā)癥和復(fù)發(fā)。本研究旨在調(diào)查胸外科住院肺癌合并慢阻肺的情況。方法回顧性分析北京大學(xué)人民醫(yī)院胸外科2015年1月-2015年12月收治的原發(fā)性非小細胞肺癌患者。通過查閱病歷獲取患者的性別、年齡、吸煙狀況、有害職業(yè)史、癥狀、胸部計算機斷層掃描(computed tomography,CT)、術(shù)后病理、出院診斷、全套肺功能及支氣管舒張試驗,當基礎(chǔ)肺功能第一秒鐘用力呼氣容積(forced expiratory volume in one second,FEV_1)占預(yù)計值70%時即行支氣管舒張試驗。結(jié)果 703例肺癌患者進行了全套肺功能測定,其中67例進行支氣管舒張試驗,62例(92.5%)符合慢阻肺診斷。接受手術(shù)治療的肺癌患者有677例,其中41例進行支氣管舒張試驗,38例(92.7%)符合慢阻肺診斷。在接受手術(shù)治療的肺癌患者中合并慢阻肺者年齡≥65歲、男性、有吸煙史和非腺癌的比例高于未合并慢阻肺者,差異有統(tǒng)計學(xué)意義(P0.05);男性和≥65歲者更易合并慢阻肺(OR:2.807-2.374,95%CI:1.101-7.157)(P0.05)。住院前僅有3例(4.3‰)診斷慢阻肺并按慢阻肺規(guī)范治療。出院時僅有5例(7.1‰)診斷慢阻肺。結(jié)論在胸外科住院肺癌患者中行常規(guī)肺功能及支氣管舒張試驗可提高肺癌合并慢阻肺的診斷;當前肺癌合并慢阻肺診斷和治療嚴重不足,需要引起胸外科醫(yī)生重視,與呼吸內(nèi)科醫(yī)生攜手共同防治慢阻肺。
[Abstract]:Background & objective Lung cancer is an important complication of chronic obstructive pulmonary disease (COPD). COPD also affects postoperative complications and recurrence in patients with lung cancer. The purpose of this study was to investigate the lung cancer complicated with COPD in thoracic surgery. Methods the patients with primary non-small cell lung cancer admitted from January 2015 to December 2015 in the Department of Thoracic surgery in people's Hospital of Peking University were retrospectively analyzed. The patient's sex, age, smoking status, harmful occupational history, symptoms, chest computed tomography (computed tomography,CT), postoperative pathology, discharge diagnosis, complete lung function and bronchodiastolic test were obtained by consulting the medical records. Bronchodiastolic test was performed when the first second forced expiratory volume (forced expiratory volume in one second,FEV_1) of the underlying lung function was 70% of the predicted value. Results 703 patients with lung cancer underwent a complete set of pulmonary function tests, of which 67 cases were performed bronchodiastolic test, 62 cases (92.5%) were in accordance with the diagnosis of COPD. There were 677 cases of lung cancer treated surgically, of which 41 cases were treated with bronchodilation test. 38 cases (92.7%) were diagnosed as chronic obstructive pulmonary disease.
