【摘要】：目的通過(guò)對(duì)慢性腎功能不全(CKD)并發(fā)社區(qū)獲得性肺炎(CAP)患者的臨床資料進(jìn)行分析,提高對(duì)該疾病臨床特點(diǎn)的認(rèn)識(shí)。方法選擇2006~2015年在重慶市第九人民醫(yī)院呼吸內(nèi)科確診CAP的216例患者,其中并發(fā)CKD的患者有116例(并發(fā)組),對(duì)照組100例患者的臨床資料,對(duì)CKD并發(fā)CAP的臨床特點(diǎn)進(jìn)行分析。結(jié)果并發(fā)組與對(duì)照組臨床癥狀比較:咳嗽(61例∶77例)、咳痰(31例∶62例)、發(fā)熱(25例∶52例)、胸痛(0例∶26例)、納差(13例∶0例),其差異均有統(tǒng)計(jì)學(xué)意義(P0.05);呼吸困難(26例∶11例,P=0.026)、惡心嘔吐(10例∶0例,P=0.002)、咯血(0例∶7例,P=0.003)為主要表現(xiàn);CURB-65≥2分(64例∶8例,P0.001),輔助檢查:白細(xì)胞(WBC)10×10~9/L(44例∶52例,P=0.037)、胸腔積液(65例∶12例,P0.001)、低蛋白血癥(57例∶36例,P=0.051)、貧血(107例∶20例,P0.001)。結(jié)論 CKD并發(fā)肺部感染患者臨床表現(xiàn)不典型,可以消化道癥狀為主,多并發(fā)高血壓、冠心病等基礎(chǔ)疾病,病情更嚴(yán)重,易并發(fā)貧血及胸腔積液。
[Abstract]:Objective to analyze the clinical data of patients with chronic renal insufficiency (CKD) complicated with community-acquired pneumonia (CAP). Methods from 2006 to 2015, 216 patients with CAP were diagnosed in Department of Respiratory Medicine, Ninth people's Hospital of Chongqing, including 116 patients complicated with CKD (complicated group) and 100 patients in control group. The clinical characteristics of CKD complicated with CAP were analyzed.
