肺部小結節(jié)經(jīng)皮穿刺CT定位技術的研究進展

【摘要】:隨著電子計算機斷層掃描(CT)技術在臨床的逐漸廣泛應用,肺部小結節(jié)診斷率明顯上升。即使應用正電子發(fā)射計算機斷層顯像(PET-CT)或者經(jīng)皮穿刺活檢等技術,準確診斷肺部結節(jié)仍面臨挑戰(zhàn)。微創(chuàng)胸腔鏡手術在肺部小結節(jié)的病理診斷和根治切除方面發(fā)揮至關重要的作用。但是由于胸腔鏡本身的局限性,術中準確找到并切除肺內(nèi)小結節(jié)是手術的難點,也成為限制胸腔鏡手術開展的因素之一。目前,胸外科領域已有多種胸腔鏡術前肺部結節(jié)定位技術,本文則對臨床上最常用的三種CT引導下經(jīng)皮穿刺肺部小結節(jié)定位技術:經(jīng)皮hookwire穿刺定位法、經(jīng)皮穿刺碘油染色法以及經(jīng)皮穿刺彈簧圈植入法進行詳細介紹。
[Abstract]:With the wide application of computerized tomography (CT) in clinical practice, the diagnostic rate of pulmonary nodules has increased significantly. Even with positron emission computed tomography (PET-CT) or percutaneous biopsy, accurate diagnosis of pulmonary nodules remains a challenge. Minimally invasive thoracoscopic surgery plays an important role in the pathological diagnosis and radical resection of pulmonary nodules. However, due to the limitations of thoracoscopic surgery, finding and removing pulmonary nodules accurately is a difficult point and one of the factors restricting the development of thoracoscopic surgery. At present, there are many kinds of localization techniques for pulmonary nodules before thoracoscopic surgery in the field of thoracic surgery. In this paper, the three most commonly used CT guided percutaneous pulmonary nodule localization techniques: percutaneous hookwire puncture localization, Percutaneous lipiodol staining and percutaneous coil implantation were introduced in detail.
