【摘要】：目的探討慢性阻塞性肺病(COPD)穩(wěn)定期患者生活質(zhì)量與下呼吸道細(xì)菌定植及炎癥反應(yīng)的相關(guān)性。方法選擇中重度COPD穩(wěn)定期患者30例。對患者進(jìn)行生活質(zhì)量(SGRQ)問卷調(diào)查,記錄各項(xiàng)得分。采集患者與志愿者痰液作為樣本,統(tǒng)一進(jìn)行細(xì)菌半定量培養(yǎng)。對受試者進(jìn)行血清分離、血常規(guī)檢查、血?dú)夥治�、肺功能檢測,各類檢查結(jié)果進(jìn)行統(tǒng)計(jì)學(xué)分析,探討生活質(zhì)量與下呼吸道細(xì)菌定植及炎癥反應(yīng)的相關(guān)性。結(jié)果經(jīng)顯微觀察其中主要的定植細(xì)菌包括肺炎鏈球菌、洛菲不動(dòng)桿菌和銅綠假單胞菌、(副)流感嗜血桿菌。血清化驗(yàn)結(jié)果顯示,含有細(xì)菌定植的患者血清,IL-8的含量高于未含有細(xì)菌定植的患者和對照組(P0.01),FEV1%預(yù)計(jì)值和FEV1/FVC%要明顯低于無細(xì)菌定植樣品(P0.05);患者SGRQ問卷調(diào)查結(jié)果,總分、癥狀得分、活動(dòng)得分、影響分的分布情況與IL-8呈正相關(guān),同F(xiàn)EV1%預(yù)計(jì)值呈負(fù)相關(guān)(P0.01)。結(jié)論 COPD穩(wěn)定期患者生活質(zhì)量較健康人差,且部分患者存在細(xì)菌定植。細(xì)菌定植患者的生活質(zhì)量較差,炎癥反應(yīng)較重,三者具有明顯的相關(guān)性。
[Abstract]:Objective to investigate the relationship between quality of life (QOL) and bacterial colonization and inflammation in lower respiratory tract in patients with chronic obstructive pulmonary disease (COPD). Methods 30 patients with moderate and severe COPD were selected. The quality of life (QOL) was investigated by (SGRQ) questionnaire, and the scores were recorded. The sputum of patients and volunteers were collected as samples and semi-quantitative culture of bacteria was carried out. Serum separation, blood routine examination, blood gas analysis, pulmonary function test, and statistical analysis were carried out to explore the correlation between quality of life and bacterial colonization and inflammation in lower respiratory tract. Results the main colonized bacteria were Streptococcus pneumoniae, Acinetobacter Lofiae, Pseudomonas aeruginosa and Haemophilus influenzae. The serum level of IL-8 in patients with bacterial colonization was significantly lower than that in patients without bacterial colonization (P0.01) and control group (P0.01). The distribution of activity score and influence score was positively correlated with IL-8 and negatively correlated with FEV1% prediction (P0.01). Conclusion the quality of life of COPD stable patients is worse than that of healthy people, and some patients have bacterial colonization. The quality of life of bacterial colonization patients was poor, and the inflammatory reaction was more serious.
