【摘要】：目的了解主動篩查方式下云南省偏遠農(nóng)村地區(qū)肺結(jié)核患病情況。方法采用多階段分層整群等比例隨機抽樣方法對6個村15歲及以上常住人口進行問卷癥狀篩查、胸部X線攝片檢查、痰涂片和痰培養(yǎng)檢查。結(jié)果 6個村共篩查9 305人,檢出活動性肺結(jié)核101例,其中新發(fā)現(xiàn)95例,已知6例�；顒有苑谓Y(jié)核患病率為1 085.44/10萬,遠高于同期疾病監(jiān)測信息報告管理系統(tǒng)報告發(fā)病率的全省平均水平(55.6/10萬)和最高縣的水平(鎮(zhèn)雄縣180.34/10萬)。7例結(jié)核合并塵肺的患者均檢自一個擁有煤礦區(qū)的村子,占該村發(fā)現(xiàn)患者數(shù)的32%。92.5%的篩查對象和60.40%的肺結(jié)核患者無咳嗽咳痰≥2周肺結(jié)核可疑癥狀。不同性別、年齡、民族、有無肺結(jié)核可疑癥狀人群的肺結(jié)核患病率差異有統(tǒng)計學(xué)意義。結(jié)論 6個村肺結(jié)核疫情嚴峻,開展主動篩查能極大提高患者發(fā)現(xiàn)的力度。男性、少數(shù)民族、60歲及以上老年人是肺結(jié)核患病的高危人群。礦區(qū)應(yīng)作為肺結(jié)核篩查的重點地區(qū)。僅靠癥狀篩查不能發(fā)現(xiàn)無肺結(jié)核可疑癥狀的隱匿患者。
[Abstract]:Objective to investigate the prevalence of pulmonary tuberculosis in remote rural areas of Yunnan province under active screening. Methods Multi-stage stratified cluster random sampling was used to screen the symptoms of questionnaire, chest X-ray, sputum smear and sputum culture in 6 villages. Results A total of 9 305 cases were screened in 6 villages, 101 cases of active pulmonary tuberculosis were detected, of which 95 cases were newly found and 6 cases were known. The prevalence rate of active pulmonary tuberculosis is 108.5.44%, It was much higher than the provincial average (55.6 / 100, 000) and the highest county (1,80.34 / 100, 000 in Zhenxiong County) of the incidence of tuberculosis and pneumoconiosis reported by the disease surveillance information management system in the same period.
