慢性阻塞性肺疾病患者腦白質(zhì)微結(jié)構(gòu)變化的初步研究

【摘要】:目的:采用基于白質(zhì)纖維束空間統(tǒng)計(jì)分析(tract based spatial statistics,TBSS)的方法研究慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者全腦白質(zhì)纖維束微觀結(jié)構(gòu)變化。方法:選取30例穩(wěn)定期COPD患者和30例性別、年齡及教育程度匹配的健康志愿者(對照組),應(yīng)用3.0 T磁共振進(jìn)行全腦DTI(diffusion tensor imaging)序列掃描。采用TBSS方法對2組被試全腦白質(zhì)纖維束的部分各向異性指數(shù)(fractional anisotropy,FA)、平均彌散率(mean diffusivity,MD)進(jìn)行基于體素的全腦非參數(shù)統(tǒng)計(jì)學(xué)比較。結(jié)果:與正常對照組相比,COPD組的胼胝體、雙側(cè)丘腦前輻射、皮質(zhì)脊髓束、左側(cè)額枕下束、左側(cè)上縱束FA值明顯下降(P0.05,TFCE校正);胼胝體、雙側(cè)額枕下束、右側(cè)丘腦前輻射、左側(cè)鉤束MD值明顯升高(P0.05,TFCE校正)。結(jié)論:相對于正常對照組,COPD患者大腦白質(zhì)纖維束出現(xiàn)多發(fā)微觀損傷,其DTI多參數(shù)改變提示與認(rèn)知損傷、情緒調(diào)節(jié)相關(guān)的腦白質(zhì)纖維髓鞘結(jié)構(gòu)完整性發(fā)生退變。
[Abstract]:Aim: to study the microstructural changes of white matter bundle in patients with chronic obstructive pulmonary disease (chronic obstructive pulmonary disease,COPD) based on spatial statistical analysis of white matter bundle (tract based spatial statistics,TBSS). Methods: 30 patients with stable COPD and 30 healthy volunteers with matched sex, age and education level (control group) were examined with 3. 0 T Mr DTI (diffusion tensor imaging) sequence. The partial anisotropy index (fractional anisotropy,FA) and the mean diffusion rate (mean diffusivity,MD) of the white matter fiber bundles in the whole brain of the two groups were compared with the voxel based nonparametric statistics by TBSS method. Results: the FA values of corpus callosum, bilateral anterior thalamic radiation, corticospinal tract, left frontal suboccipital tract, left superior longitudinal tract, corpus callosum, bilateral frontal suboccipital tract and right thalamus were significantly decreased in COPD group compared with normal control group (P 0.05). The MD value of the left hook bundle was significantly increased (P 0.05). Conclusion: compared with the normal control group, there are multiple microcosmic lesions in the white matter fiber bundle of the brain, and the changes of multiple parameters of DTI suggest that the myelin structure integrity of the white matter fiber is related to cognitive impairment and emotion regulation.
