【摘要】：目的探討細(xì)胞免疫功能與老年細(xì)菌性肺炎及其嚴(yán)重程度的相關(guān)性。方法選擇70歲以上老年細(xì)菌性肺炎患者120例作為觀察組,70歲以上老年健康體檢者120例作為對(duì)照組,觀察組根據(jù)肺炎嚴(yán)重程度分為重癥肺炎組71例,非重癥肺炎組49例。采用流式細(xì)胞儀分析細(xì)胞表面CD3~+、CD4~+、CD8~+陽(yáng)性細(xì)胞率,CD4~+/CD8~+,自然殺傷細(xì)胞(NK)活性,采用酶聯(lián)免疫吸附試驗(yàn)(ELISA)測(cè)定血清腫瘤壞死因子(TNF)-α、白細(xì)胞介素(IL)-6、IL-8水平。結(jié)果觀察組CD8~+明顯高于對(duì)照組(P0.05),CD4~+、CD4~+/CD8~+、NK活性明顯低于對(duì)照組(P0.05),兩組CD3~+無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。觀察組血清TNF-α、IL-6、IL-8水平均明顯高于對(duì)照組(P0.05)。重癥肺炎組CD4~+、CD8~+、CD4~+/CD8~+、NK活性明顯低于非重癥肺炎組(P0.05),兩組CD3~+無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。重癥肺炎組血清TNF-α、IL-6、IL-8水平明顯高于非重癥肺炎組(P0.05)。結(jié)論老年細(xì)菌性肺炎患者存在細(xì)胞免疫功能異常,隨著病情嚴(yán)重程度的增加,細(xì)胞免疫功能下降更顯著。
[Abstract]:Objective to investigate the relationship between cellular immune function and the severity of bacterial pneumonia in the elderly. Methods 120 patients over 70 years old with bacterial pneumonia were selected as the control group. According to the severity of pneumonia, the observation group was divided into severe pneumonia group (71 cases) and non-severe pneumonia group (49 cases). The (NK) activity of natural killer cells was measured by flow cytometry. The levels of serum tumor necrosis factor (TNF)-偽 and interleukin (IL)-6 (IL-8) were measured by Elisa (ELISA). Results the activity of CD8 ~ + NK in the observation group was significantly higher than that in the control group (P0.05). The NK activity of CD4 ~ / CD4 ~ / CD8 ~ + in the observation group was significantly lower than that in the control group (P0.05), but there was no significant difference between the two groups (P0.05). The serum levels of TNF- 偽 IL-6 and IL-8 in the observation group were significantly higher than those in the control group (P0.05). The NK activity in severe pneumonia group was significantly lower than that in non-severe pneumonia group (P0.05), but there was no significant difference between the two groups (P0.05). The level of serum TNF- 偽 IL-6 + IL-8 in severe pneumonia group was significantly higher than that in non severe pneumonia group (P0.05). Conclusion there is abnormal cellular immune function in senile patients with bacterial pneumonia, and the decrease of cellular immune function is more significant with the increase of severity of the disease.
