【摘要】：目的:探討呼吸內(nèi)科臨床藥師參與藥物治療方案的制訂與實(shí)施藥學(xué)監(jiān)護(hù)的方法。方法:臨床藥師參與呼吸內(nèi)科1例侵襲性肺曲霉菌病并大咯血的治療過程中,建議去氨加壓素替代垂體后葉素用于大咯血的治療并實(shí)施藥學(xué)監(jiān)護(hù),分析去氨加壓素用于治療咯血的依據(jù)和導(dǎo)致患者出現(xiàn)少尿的原因及治療。結(jié)果:臨床藥師的建議被采納,患者的咯血癥狀明顯好轉(zhuǎn),病情得到有效控制。去氨加壓素引起患者尿量減少后給予呋塞米對抗治療,尿量恢復(fù)正常。結(jié)論:去氨加壓素可以作為垂體后葉素治療大咯血的替代藥物,短時間內(nèi)引起少尿,出現(xiàn)此情況后可以應(yīng)用袢利尿劑加以糾正。
[Abstract]:Objective: to explore the method of pharmaceutical monitoring for clinical pharmacists in respiratory medicine department. Methods: clinical pharmacists participated in the treatment of invasive pulmonary aspergillosis with massive hemoptysis in a case of respiratory medicine. It was suggested that desmopressin should be used instead of pituitrin in the treatment of massive hemoptysis and pharmacological monitoring should be carried out. To analyze the basis of deaminopressin in the treatment of hemoptysis and the causes and treatment of oliguria in patients. Results: the clinical pharmacist's suggestion was adopted, the patient's hemoptysis symptom obviously improved, the condition was effectively controlled. The urine volume returned to normal after treatment with furosemide. Conclusion: desaminopressin can be used as a substitute for hypophysin in the treatment of massive hemoptysis, which can cause oliguria in a short time, and can be corrected by loop diuretic.
