【摘要】：目的探討叩擊支氣管解剖位置進(jìn)行排痰對預(yù)防住院呼吸內(nèi)科患者肺部感染的效果及應(yīng)用價值。方法選取2014年4月-2016年6月于醫(yī)院治療的124例住院呼吸內(nèi)科排痰困難患者,按照隨機(jī)表法將研究對象分為觀察組和對照組,每組62例,對照組患者進(jìn)行傳統(tǒng)叩背排痰法,觀察組患者使用支氣管解剖位置叩擊排痰法,在排痰完后對患者進(jìn)行吸痰處理,記錄兩組患者前5天的排痰量,比較兩組患者排痰的有效率及比較兩組患者吸痰后血?dú)庵笜?biāo)的變化情況。結(jié)果對照組與觀察組患者前5天排痰量差異有統(tǒng)計學(xué)意義(P0.05);對照組患者排痰有效43例,有效率69.4%,觀察組患者排痰有效56例,有效率90.3%,兩組患者差異有統(tǒng)計學(xué)意義(P0.05);對照組與觀察組患者吸痰前血?dú)庵笜?biāo)差異無統(tǒng)計學(xué)意義,兩組患者吸痰后差異有統(tǒng)計學(xué)意義(P0.05);對照組患者肺部感染情況:銅綠假單胞菌感染5例(8.1%)、肺炎克雷伯菌3例(4.8%)、白念珠菌4例(6.5%)、曲霉菌屬3例(4.8%),總肺部感染例數(shù)15例(24.2%),觀察組患者肺部感染情況:銅綠假單胞菌2例(3.3%)、肺炎克雷伯菌1例(1.6%)、白念珠菌1例(1.6%),總肺部感染例數(shù)4例(6.5%),兩組患者肺部感染差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論根據(jù)支氣管解剖位置叩擊排痰比傳統(tǒng)叩擊排痰法更有效果,排痰的有效率更高,有利于患者恢復(fù),能夠有效預(yù)防住院呼吸內(nèi)科患者肺部感染。
[Abstract]:Objective to explore the effect and application value of sputum removal from percussion bronchus in preventing pulmonary infection of inpatients with respiratory medicine. Methods 124 patients with difficulty in sputum drainage from April 2014 to June 2016 were divided into observation group (n = 62) and control group (n = 62). The patients in the observation group were treated with sputum aspiration after sputum drainage by percussion at the anatomic position of the bronchus, and the amount of sputum excretion in the first 5 days of the two groups was recorded. To compare the effective rate of sputum excretion and the changes of blood gas index after sputum aspiration in the two groups. Results there was a significant difference in the amount of sputum excretion between the control group and the observation group in the first 5 days (P0.05), the effective rate of sputum drainage was 69.4 in 43 patients in the control group, and 56 patients in the observation group. The effective rate was 90.3%, the difference between the two groups was statistically significant (P0.05), there was no significant difference in blood gas index between the control group and the observation group before sputum aspiration. There was significant difference between the two groups after sputum aspiration (P0.05), the pulmonary infection in the control group was: Pseudomonas aeruginosa infection in 5 cases (8.1%), Klebsiella pneumoniae in 3 cases (4.8%), Candida albicans in 4 cases (6.5%), Aspergillus in 3 cases (4.8%), total pulmonary infection in 15 cases. There were 2 cases of Pseudomonas aeruginosa (3.3%), 1 case of Klebsiella pneumoniae (1.6%), 1 case of Candida albicans (1.6%) and 4 cases (6.5%) of total pulmonary infection. Conclusion according to the anatomical position of the bronchus, percussion is more effective than the traditional percussion method, and the effective rate of sputum drainage is higher, which is beneficial to the recovery of the patients and can effectively prevent the pulmonary infection of the patients in the department of respiratory medicine.
