【摘要】：目的通過對12例確診肺曲霉菌病患者的臨床特征、宿主因素、CT特點、診斷及治療的回顧性分析,以提高早期診斷和治療水平。方法采用回顧性研究,查閱2004年1月-2011年9月新疆醫(yī)科大學第一附屬醫(yī)院病歷資料,收集肺曲霉菌病確診病歷相關資料,制作調查表格。結果 12例患者中侵襲性肺曲霉病2例,均有宿主因素,臨床特征以發(fā)熱、呼吸急促為主,胸部CT主要表現為結節(jié)、實變、斑片及空洞影,1例出現新月征,1例支氣管鏡下取痰曲霉菌屬培養(yǎng)陽性,1例肺組織病理檢查陽性,1例抗真菌治療有效,1例死亡;寄生性肺曲霉菌病10例,8例有肺部基礎疾病,臨床癥狀以咯血多見,胸部CT表現結節(jié)8例,團塊、空洞、斑片各2例,暈輪征、新月征各1例;9例行手術治療,其中6例在術前未被考慮肺曲霉菌病;術后電話隨訪7例,治愈6例。結論肺曲霉菌病臨床特征不典型,痰檢陽性率低,早期不易診斷,另外部分患者因病情危重不能耐受手術或氣管鏡檢查;其早期診斷方法有待進一步探討。
[Abstract]:Objective to improve the early diagnosis and treatment of pulmonary aspergillosis by retrospective analysis of clinical features, host factor CT features, diagnosis and treatment of 12 patients with pulmonary aspergillosis. Methods retrospective study was used to review the medical records of the first affiliated Hospital of Xinjiang Medical University from January 2004 to September 2011. The relevant data of the confirmed cases of pulmonary aspergillosis were collected and the investigation forms were made. Results two of 12 patients with invasive pulmonary aspergillosis had host factors. The clinical features were fever and shortness of breath. Crescent sign in 1 case and crescent sign in 1 case, sputum aspergillus culture positive in 1 case, pulmonary histopathology in 1 case, antifungal therapy in 1 case, parasitic pulmonary aspergillosis in 10 cases, pulmonary basic disease in 8 cases, The clinical symptoms were hemoptysis in 8 cases, mass in 8 cases, cavity in 2 cases, plaque in 2 cases, halo sign in 2 cases, crescent sign in 1 case and crescent sign in 9 cases, in which 6 cases were not considered pulmonary aspergillosis before operation, 7 cases were followed up by telephone after operation. 6 cases were cured. Conclusion the clinical features of pulmonary aspergillosis are atypical, the positive rate of sputum examination is low, and the early diagnosis is difficult to diagnose.
