在治療急性肺血栓栓塞過(guò)程中對(duì)心肌細(xì)胞缺血缺氧保護(hù)的實(shí)驗(yàn)研究

【摘要】:目的:研究左卡尼汀在治療急性肺血栓栓塞過(guò)程中對(duì)心肌細(xì)胞缺血缺氧的治療和保護(hù)的作用. 方法:通過(guò)造模,造成SD大鼠急性肺血栓栓塞,并進(jìn)行溶栓,在常規(guī)溶栓過(guò)程中,,加用左卡尼汀,觀察該藥物在治療急性肺血栓栓塞過(guò)程中對(duì)心肌細(xì)胞所起的作用. 結(jié)果:急性肺血栓栓塞(PTE)為急性發(fā)病,且致死率極高, PTE患者會(huì)不同程度的伴發(fā)心肌細(xì)胞的缺血缺氧,類(lèi)似急性心肌梗死,溶栓可恢復(fù)受損的肺組織細(xì)胞,但心肌細(xì)胞的恢復(fù)卻效果很差,本實(shí)驗(yàn)中,溶栓過(guò)程中加入左卡尼汀,既恢復(fù)受損的肺組織,而且使心肌細(xì)胞缺血缺氧的狀態(tài)得到了緩解.很大程度上促進(jìn)了溶栓治療的效果. 結(jié)論:急性肺血栓栓塞,伴發(fā)心肌細(xì)胞的缺血缺氧,同時(shí)出現(xiàn)類(lèi)似急性心肌梗死的心電圖表現(xiàn)和cTn改變,溶栓同時(shí)加用左卡尼汀,溶栓結(jié)束后,測(cè)量cTn,其明顯比單用溶栓藥有所降低.因此,可以證明左卡尼汀在治療急性肺血栓栓塞過(guò)程中有重要臨床意義.
[Abstract]:Aim: to study the protective effect of levacarnitine on myocardial ischemia and hypoxia in the treatment of acute pulmonary thromboembolism. Methods: acute pulmonary thromboembolism was induced and thrombolytic therapy was carried out in SD rats. In the course of routine thrombolytic therapy, levacarnitine was added to observe the effect of the drug on cardiomyocytes during the treatment of acute pulmonary thromboembolism. Results: acute pulmonary thromboembolism (PTE) is an acute disease with high mortality. Pte patients may be accompanied by myocardial ischemia and hypoxia in varying degrees, similar to acute myocardial infarction. Thrombolytic therapy can restore damaged lung tissue cells. However, the effect of cardiomyocyte recovery was very poor. In this experiment, levocarnitine was added to the thrombolytic process, which not only recovered the damaged lung tissue, but also alleviated the ischemia and hypoxia of myocardial cells. To a large extent, the effect of thrombolytic therapy was promoted. Conclusion: acute pulmonary thromboembolism, accompanied by ischemia and hypoxia of cardiac myocytes, accompanied by changes of ECG and cTn similar to acute myocardial infarction, thrombolytic therapy combined with L-carnitine, after thrombolytic therapy, The measurement of cTnwas significantly lower than that of single thrombolytic agent. Therefore, it can be proved that levacarnitine has important clinical significance in the treatment of acute pulmonary thromboembolism.
