【摘要】：目的:對(duì)比分析低濃度對(duì)比劑結(jié)合低劑量寶石CT灌注及增強(qiáng)成像在孤立性肺結(jié)節(jié)(SPN)良惡性鑒別診斷中的應(yīng)用價(jià)值。方法:對(duì)46例孤立性肺結(jié)節(jié)患者行寶石CT低劑量增強(qiáng)加灌注掃描,其中惡性34例(A組),良性12例(B組)。比較兩組結(jié)節(jié)血流量(BF)、血容量(BV)、平均通過時(shí)間(MTT)、表面通透性(PS)以及增強(qiáng)前后的結(jié)節(jié)強(qiáng)化值、結(jié)節(jié)-主動(dòng)脈強(qiáng)化值比(S/A)的差異。結(jié)果:良、惡性SPN的BV值及PS值的差異均有統(tǒng)計(jì)學(xué)意義(P值分別為0.009和0.003),BF值及MTT值的差異無統(tǒng)計(jì)學(xué)意義(P值分別為0.889和0.149);以BV≥7ml/100g聯(lián)合PS≥7ml/100g/min較單獨(dú)以BV或PS作為診斷惡性結(jié)節(jié)的閾值時(shí)的敏感性、特異性、陽性預(yù)測值、陰性預(yù)測值和診斷符合率均增高,分別為96.6%、64.7%、82.4%、91.7%、84.8%。良、惡性SPN的結(jié)節(jié)強(qiáng)化值及結(jié)節(jié)-主動(dòng)脈強(qiáng)化值比的差異有統(tǒng)計(jì)學(xué)意義(t值分別為0.004和0.022);以結(jié)節(jié)強(qiáng)化值≥15HU、結(jié)節(jié)-動(dòng)脈強(qiáng)化值比≥10%作為惡性結(jié)節(jié)的診斷閾值,靈敏度、特異度、陽性預(yù)測值、陰性預(yù)測值依次為94.1%、91.7%、96.97%和84.6%。結(jié)論:以PS≥7ml/100g/min聯(lián)合結(jié)節(jié)強(qiáng)化值≥15HU、結(jié)節(jié)-動(dòng)脈強(qiáng)化值比≥10%作為惡性結(jié)節(jié)診斷閾值時(shí),有利于SPN良、惡性的鑒別,在CT灌注及增強(qiáng)成像診斷惡性SPN的準(zhǔn)確性相近時(shí),應(yīng)選擇診斷敏感度較高的BV值。
[Abstract]:Objective: to compare and analyze the value of low concentration contrast medium combined with low dose gemstone CT perfusion and enhanced imaging in the differential diagnosis of benign and malignant solitary pulmonary nodules (SPNs). Methods: Forty-six patients with solitary pulmonary nodules were examined with low dose enhanced sapphire CT and perfusion scanning. 34 cases of malignant pulmonary nodules were found in group A and 12 cases in benign group B.
