【摘要】:目的探討深吸氣量(IC)、深吸氣量與肺總量之比(IC/TLC)(或稱吸氣分?jǐn)?shù),IF)及相關(guān)肺功能指標(biāo)與穩(wěn)定期中重度COPD患者運(yùn)動(dòng)耐力的相關(guān)性。方法對(duì)42例處于穩(wěn)定期的中重度COPD患者及34例同年齡段健康志愿者進(jìn)行常規(guī)肺功能檢測(cè)(PFT)及斜坡式功率遞增癥狀限制性心肺運(yùn)動(dòng)試驗(yàn)(CPET),測(cè)定相關(guān)肺通氣功能參數(shù)、氣體交換參數(shù),并記錄受試者CPET的終止原因。結(jié)果 (1)COPD患者的IF與運(yùn)動(dòng)耐力(峰值攝氧量占預(yù)計(jì)值的百分比,peak VO2%pred)有顯著的相關(guān)性(r=0.74,p0.001),IF可作為預(yù)估COPD患者運(yùn)動(dòng)耐力減低的獨(dú)立變量,并且其敏感度及特異度均高于FEV1%pre,多重線性回歸模型方程為:peak VO2%pred=94.85IF+0.4FEV1%pred+29.2(RC2=0.61,P0.001),(2)低IF值(0.23)的COPD患者(13例)比高IF值(≥0.23)COPD患者(29例)有著更嚴(yán)重的肺過度充氣(IC%pred為(51.25±9.46)%對(duì)(79.38±17.22)%;P0.05)及運(yùn)動(dòng)耐力下降[peak VO2%pred為(56.0±6.23)%對(duì)(84.52±10.05)%,P0.01),(3)低IF值(0.23)與高IF值(≥0.23)的COPD患者在峰值運(yùn)動(dòng)時(shí)的呼吸頻率[peakBF為(31.62±7.1)對(duì)(32.21±6.3)breaths/min,P0.05],無顯著性差異的情況下,前者的峰值分鐘通氣量(peak VE)及峰值潮氣量(peak VT)均較后者下降[(35.62±8.10)對(duì)(49.14±12.59)L/min,(1.11±0.25)對(duì)(1.57±0.35)L,P0.05];低IF值(0.23)的COPD患者中有11例(84.6%)因呼吸困難終止運(yùn)動(dòng),高IF值(≥0.23)COPD患者中有12例(41.4%)因呼吸困難終止運(yùn)動(dòng),13例(44.8%)因腿酸乏力停止運(yùn)動(dòng),兩組間終止運(yùn)動(dòng)原因的構(gòu)成比差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論吸氣分?jǐn)?shù)較FEV1可更好的反映肺過度充氣及預(yù)估穩(wěn)定期中重度COPD患者的運(yùn)動(dòng)耐力。
[Abstract]:Objective to investigate the correlation between the ratio of deep inspiratory volume and total lung volume and the relative pulmonary function and exercise endurance in patients with moderate and severe COPD at stable stage. Methods 42 moderate and severe COPD patients at stable stage and 34 healthy volunteers of the same age were examined by routine pulmonary function test (PFTs) and slope power increase symptom restricted cardiopulmonary exercise test (CPETT), and the relevant pulmonary ventilation parameters were measured. Gas exchange parameters were recorded and the cause of termination of CPET was recorded. Results there was a significant correlation between if and exercise endurance (peak oxygen uptake as a percentage of predicted value) in patients with COPD.
