支氣管哮喘急性發(fā)作患者血清IL-37水平變化的探索性研究

【摘要】:背景:支氣管哮喘(bronchial asthma,簡稱哮喘)以多種炎癥基因表達(dá)增加或異常表達(dá)為特征,是由嗜酸性粒細(xì)胞等多種炎癥細(xì)胞和炎癥介質(zhì)及細(xì)胞組分參與的一種氣道慢性炎癥疾病。全世界約有3億人飽受哮喘病的折磨,哮喘給人類帶來了顯著的社會和經(jīng)濟負(fù)擔(dān)。研究哮喘病防治及發(fā)病機制已成為世界衛(wèi)生組織的重要課題。 白介素-37(IL-37)是IL-1家族中的細(xì)胞因子,,是一種天然地炎癥調(diào)節(jié)因子,在機體免疫反應(yīng)過程中起到抗炎作用,因此,作為抗炎細(xì)胞因子而逐漸受到重視。目前研究發(fā)現(xiàn)IL-37在多種疾病中均起到保護(hù)作用,特別與肝炎、休克、腸道炎癥以及病態(tài)肥胖等炎癥和自身免疫性疾病等關(guān)系密切。支氣管哮喘作為免疫炎癥性疾病,其發(fā)病機制可能與IL-37密切相關(guān),但目前國內(nèi)外尚無相關(guān)研究報導(dǎo)。 目的:探討IL-37在支氣管哮喘患者機體內(nèi)的變化,進(jìn)一步為哮喘的免疫干預(yù)治療開辟新領(lǐng)域。 方法:篩選支氣管哮喘急性發(fā)作期患者50例作為觀察組,并通過血清IgE水平將其分為IgE陽性組25例及IgE陰性組25例。采用多索茶堿、孟魯司特鈉以及沙美特羅氟替卡松治療2周。測定治療前、后患者血清中IL-37水平及肺功能變化情況。選取20例正常人作為健康對照組,測定其血清中IL-37水平及肺功能。對比各組間各項指標(biāo)變化有無統(tǒng)計學(xué)意義。 結(jié)果:1.與健康對照組相比,哮喘急性發(fā)作期患者血清中IL-37水平有降低趨勢,差異有統(tǒng)計學(xué)意義(P 0.05); 2.IgE陽性組哮喘患者血清IL-37水平低于IgE陰性組患者,差異有統(tǒng)計學(xué)意義(P 0.05); 3.治療后患者血清IL-37水平與治療前相比有所升高,差異有統(tǒng)計學(xué)意義(P 0.05); 4.支氣管哮喘患者血清IL-37水平與PEF%、FEV1/FVC%變化呈正相關(guān)(P 0.05)。 結(jié)論:IL-37作為保護(hù)性炎癥細(xì)胞因子參與支氣管哮喘的發(fā)病過程,同時IL-37與IgE陽性的哮喘患者關(guān)系尤為密切。IL-37與哮喘的嚴(yán)重程度具有相關(guān)性,病情越重,血清IL-37水平越低。
[Abstract]:Background: bronchial asthma (asthma) is characterized by increased or abnormal expression of a variety of inflammatory genes. It is a chronic airway inflammatory disease involving many inflammatory cells, inflammatory mediators and cell components, such as eosinophils. About 300 million people all over the world suffer from asthma, and asthma is brought to human beings. Significant social and economic burden. Studying asthma control and pathogenesis has become an important issue of WHO.
Interleukin -37 (IL-37), a cytokine in the IL-1 family, is a natural inflammatory regulator and plays an anti-inflammatory role in the immune response of the body. Therefore, it has been paid more and more attention as an anti-inflammatory cytokine. At present, it is found that IL-37 plays a protective role in many diseases, especially with hepatitis, shock, intestinal inflammation and disease. Obesity and other inflammatory diseases are closely related to autoimmune diseases. As an immune inflammatory disease, the pathogenesis of bronchial asthma is closely related to IL-37, but there is no related research at home and abroad.
Objective: To explore the changes of IL-37 in the body of patients with bronchial asthma, and to further open up a new field for immunotherapy of asthma.
Methods: 50 cases of acute episode of bronchial asthma were selected as observation group, and 25 cases of IgE positive group and 25 cases of IgE negative group were divided into 25 cases and 25 cases of IgE negative group. The level of IL-37 and the change of lung function in the blood clear before and after treatment were measured. 20 Normal subjects were used as healthy controls. Serum IL-37 levels and lung function were measured.
Results: 1. compared with healthy controls, the serum IL-37 level in patients with acute exacerbation of asthma decreased, and the difference was statistically significant (P 0.05).
The serum IL-37 level of asthmatic patients in 2.IgE positive group was lower than that in IgE negative group (P 0.05).
3. after treatment, the serum IL-37 level increased compared with that before treatment, and the difference was statistically significant (P 0.05).
4. there was a positive correlation between serum IL-37 level and PEF% and FEV1/FVC% in patients with bronchial asthma (P 0.05).
Conclusion: IL-37 is involved in the pathogenesis of bronchial asthma as a protective inflammatory cytokine. At the same time, the relationship between IL-37 and IgE positive asthmatic patients is particularly closely related to the severity of.IL-37 and the severity of the disease, the lower the level of serum IL-37.
