【摘要】：目的討急性呼吸窘迫綜合征患者應(yīng)用無創(chuàng)通氣治療成功與否及預(yù)后的影響因素。方法以2012年6月至2015年5月本院收治的96例急性呼吸窘迫綜合征(ARDS)患者為研究對象,其中入院時使用無創(chuàng)通氣(NIV)治療的患者56例,直接氣管插管的患者40例,分析ARDS患者無創(chuàng)通氣治療成功與否及預(yù)后的影響因素。結(jié)果氧合指數(shù)、膿毒性休克比例和ARDS嚴(yán)重度是影響NIV治療成功與否的因素。APACHE II評分、氧合指數(shù)、膿毒性休克比例和ARDS嚴(yán)重度是ARDS患者死亡的危險因素。結(jié)論無創(chuàng)通氣更適合于輕度ARDS患者的治療,ARDS患者APACHE II評分、膿毒性休克比例越高,氧合指數(shù)越低,病情越嚴(yán)重時,其死亡率則越高。
[Abstract]:Objective to investigate the success and prognostic factors of non-invasive ventilation in patients with acute respiratory distress syndrome (ARDS). Methods from June 2012 to May 2015, 96 patients with acute respiratory distress syndrome (ARDS) were studied, including 56 patients treated with NIV and 40 patients with direct tracheal intubation. To analyze the factors influencing the success and prognosis of non-invasive ventilation in patients with ARDS. Results Oxygenation index, septic shock rate and severity of ARDS were the factors influencing the success of NIV treatment. Apache II score, oxygenation index, septic shock ratio and ARDS severity were the risk factors of death in ARDS patients. Conclusion Non-invasive ventilation is more suitable for APACHE II score of mild ARDS patients. The higher the proportion of septic shock, the lower the oxygenation index and the higher the death rate.
