【摘要】：目的探討雙腔中心靜脈導(dǎo)管經(jīng)皮穿刺置入治療老年肺膿腫的臨床療效和安全性。方法選取該院呼吸科收治的42例老年肺膿腫患者,按照隨機(jī)數(shù)字表法分為觀察組和對(duì)照組各21例。兩組患者均給予常規(guī)的治療方案,對(duì)照組在常規(guī)治療方案的基礎(chǔ)上加用纖維支氣管鏡膿腫腔灌洗治療;觀察組加用雙腔中心靜脈導(dǎo)管經(jīng)皮穿刺置入,并對(duì)膿腫腔進(jìn)行反復(fù)沖洗、抽吸清除膿液。對(duì)比兩組患者臨床療效、住院天數(shù)、發(fā)熱天數(shù)以及并發(fā)癥的發(fā)生情況。結(jié)果觀察組第3周痊愈率和顯效率分別為52.38%和80.95%,出院時(shí)(≥第4周)的痊愈率和顯效率分別為95.24%和95.24%,均顯著高于對(duì)照組(19.05%和42.86%,52.38%和61.90%)(P0.05)。觀察組并發(fā)癥的發(fā)生率為19.05%(4/21),對(duì)照組為33.33%(7/21),兩組比較沒(méi)有統(tǒng)計(jì)學(xué)差異(P0.05)。觀察組患者的發(fā)熱(37.5℃)天數(shù)和住院天數(shù)均顯著低于對(duì)照組(P0.05)。結(jié)論雙腔中心靜脈導(dǎo)管經(jīng)皮穿刺置入治療老年肺膿腫具有良好的臨床療效,且安全性高,能夠顯著縮短患者的住院天數(shù),值得在臨床推廣。
[Abstract]:Objective to evaluate the clinical efficacy and safety of percutaneous transluminal double cava catheter placement in elderly patients with pulmonary abscess.
